following dates January 25, 26, 27, February 1, 2, 3, 6, 7, 8, 9, 10, 13, 14, 15, 2023 from 7:30AM-5:00PM.

I have considered the benefits that my son or daughter will derive from his/her participation in this
activity provided that due care and precaution will be observed to ensure the comfort and safety of my
child DepEd employees and personnel may not be held responsible for any untoward incident that may
happen beyond their control.
