With due respect it is stated , I Farnaz kefayayi having Iranian nationality , recently
graduated from department of pharmacy . I have to move to Karachi as how I have to
evacuate the hostel regarding policy .The related Pharm-D degree which I have applied its in
process and I have no one here to receive my degree along with photocopy attested for
applying to Punjab pharmacy counsil . So I want to hand over this responsibility to Madeeha
Yaqoob , Superintendent,BS Section to do this on my behalf and post my degree with
photocopy attested to Karachi on my adress that I will provide to her .

So , I want to inform and ask for the permission of respected chairman of pharmacy
department regarding this .
