Reason and Source of Referral:
The child was referred by his mother for IQ assessment with complaints of speech and
behavioral related issues.
Behavioral Observation:
Child was dressed up properly and appeared to have satisfactory personal hygiene. He seems to
be initially confused, shy and his voice pitch was also low. Eye contact was partially maintained
and command following was good. The child has recognition of just two shapes (circle and
Triangle). He showed satisfactory response on educational activities like puzzle solving and
different kinds of sorters (Animals and fruits sorter). During difficult tasks he showed
anxiousness i.e. moving hands and bite nails. He also faces difficulty in social interaction. It was
also observed that he has problem in describing the information related to academics and general
discussion. He has unsatisfactory performance in reading and writing skills according to his
academic grade level.
Case History:
The history reported by mother reveals that the child was a case of normal delivery. First cry was
present and color was white. The weight at the time of birth was 4 pounds. The child could not
hold things properly. He has sleep disturbance since his childhood. Self talk was reported by his
mother. The child does not like to interact with people. His relationship with family members
was satisfactory.
Details of Developmental Milestones are as follows:
Neck holding = 4 months
Sitting = 9 months
Crawling = 1 year
Standing = 1 year and 2 months
Walking = 1 year and 6 months
Speech = 5 years (1st word)
= (Currently 5 to 6 words sentence)
Results of Test Administration:
Portage Guide to Early Education (PGEE)
Portage for Early Education (PGEE) administered for taking developmental profile.

Slosson Drawing Coordination Test (SDCT):
The child’s score on SDCT is 36% which indicate that his eye hand coordination does not seems
to be intact.
Clinical Assessment of Behavior Scale (CABS):
The scores of Clinical Assessment of Behavior Scale (CABS) reveals that his age equivalence in
Language Development lies within 6 years, Independent Functioning 6 years, Family Role
Performance 7 years, Economical Vocational Activity 6 years and in Socialization it lies within 7
years. The age equivalent for the Adaptive Behavior Composite is 6 year.
Slosson Intelligence Test (SIT):
The client score on SIT was 33 and IQ range lies within (29-37). His chronological age (CA) is
72 and mental age (MA) is 217.
Standard Progressive Matrices (SPM):
The child has got 14 score on SPM test he falls in category of grade V score shows that the child
seems to be Intellectually Defective.
Human Figure Drawing (HFD)
The HFD of the child shows that he seems to have Anxiety, with drawl, shyness, overt
aggressiveness, refusal to communicate with others, poor inner control, guilt and tendency to
withdraw when faced with over whelming power.
Speech Evaluation:
The child has not age appropriate speech development which contains completely intelligible and
socially useful speech with all vowels and consonants. Pre speech skills of the child were intact
as eye contact was maintained. On seat behavior, command following and imitation was good.
Oral motor mechanism (OMM) of the child was intact as he has satisfactory movement of the
articulators like lips, tongue, jaw etc. Receptive language of the child was not satisfactory which
was assessed by where type of questions. Expressive language was also not satisfactory which
was assessed by what type of questions. It was observed that the child has hyper nasality of
voice. It was also observed that he was replacing most of the phonemes with other phonemes like
t/k, d/g, t/j etc. The child seems to have Delayed Speech and Language development.
Conclusion:
The formal and informal assessment reveals that the child seems to have Moderate Level of
Intellectual Disability. Moreover, it was also identified that he has difference between the age
equivalent adaptive functioning. He also seems to have delayed speech and language
development.
Suggestions:
Psycho education and Counseling of parent regarding child’s problem.
Speech therapy is strongly recommended for his speech and language problem.
Individualized Psychological Therapeutic sessions should be conducted on regular basis
to overcome his problems and study work should be based upon on Individualized
Educational Plan (IEP’s).
Extra Tutorial help needs to provide and focus should be on his problematic area.
Reinforcement techniques and behavioral management can be effective in dealing with
problems related to learning, behavior and emotional issues.
Vocational Training should also recommend.
Re-assessment of the child is recommended after 6 months administration of Therapeutic
Sessions.
