Sir:
The undersigned, a bonafide member of the Philippine National Police
presently assigned at La Libertad Police Station, this Provincial Police Office,
respectfully request for an Optional Retirement from the PNP service effective June
1, 2024.
That I will be accumulating Twenty-Nine (29) years, and One Fourty Seven
(147) days of honorable police services on the effectivity date of my retirement, as
requested.
That after all these years in the service, I have decided to retire and return
to a full civilian life.
That this request is not motivated by ill-feeling toward the PNP organization
or any misunderstanding with my co-policemen.
My mailing address upon retirement is Sitio Cotcot, Purok 1, Brgy Basak,
Guihulngan City, Negros Oriental.
I earnestly hope that this request be given due consideration and immediate
approval.

AFFIDAVIT OF IRREVOCABLE UNDERTAKING TO AVAIL OF
OPTIONAL RETIREMENT
I, PEMS John Eric Carvajal Cordovez, with badge number 100041, of legal
age, married, a bonafide member of the Philippine National Police, with residence
and postal address at National Highway, Sitio Cotcot, Purok 1, Brgy Basak,
Guihulngan City, Negros Orientral after having been duly sworn to in accordance
with law, do hereby depose and say:
1. That I have filed my application for Optional Retirement from the PNP
service pursuant to Section 40, RA 6975;
2. That I am applying for Optional Retirement effective on June 1, 2024;
3. That I am submitting all required pertinent documents to support my
application;
4. That I bind myself to the irrevocability of this undertaking and would
not ask for any cancellation of any order issued for the purpose;
5. That I am executing this affidavit freely and voluntarily to support the
processing of my application for Optional Retirement and the issuance
of the corresponding retirement order effective on the date requested.

and say:
That I am applying for optional retirement from PNP service effective June
1, 2024;
That I have existing lo9an(s) with PSSLA, AFPSLAI AFP-MBAI AND
AWMSLAI.
That I am authorizing the Retirement Claims and Funds Management
Division (RCFMD) and/of Pension and Gratuity Division (PDG) OF THE PNP
Retirement and Benefits Administration Service (PRBS) to deduct the total
amount I owe the above Financial Institution(s) from my Commutation of
Accumulated Leave (CAL) claims and/or Lump Sum/Retirement Gratuity
claims.
That I am executing this Affidavit to the truthfulness of the foregoing facts
and for whatever legal purpose this may serve.
