1. Green and red scaffold tags shall be used, where applicable, to inform /
warn users about the condition or readiness of a scaffold
2 Sufficient number of red tags (i.e. every after two bays and on all access
points) shall be installed during construction of lengthy scaffolds
3 Only competent and authorized personnel shall be delegated the task to
design, erect, alter, modify, dismantle, inspect and maintain scaffolding in
KNPC premises.
4 The scaffoldings erected in the process units shall be provided with
‘reflector tape’ for clear visibility at man height level in darkness/foggy
weather as safeguard against hits and bangs.
5 Vertical and perimeter nettings are to be provided where there is scaffold
erection as there is possibility of falling or flying objects injuring people and
damaging property
6 Illumination levels while erecting scaffolding, particularly on confined spaces
shall meet K-EPA standard on ‘minimum light level’ (i.e. 100 – 300 lux)
required for occupational activities.
7 Personal fall arrest systems on scaffold erection and dismantling should
have the scaffolder’s lanyards attached to either a vertical lifeline, horizontal
lifeline, or stable scaffold structure member
8 Job engineer shall evaluate and ensure compatible personal fall arrest
components are on-use
9 Only competent and authorized personnel (i.e. Scaffold Engineers) shall be
delegated the task to install, inspect and maintain Safety Net System in
KNPC premises.
10 When safety nets are used as personnel fall protection, it shall be installed
as close as practicable under the walking/working surface on which
employees are working, but in no case more than 30 feet (9.1 m) below
such level. When nets are used on bridges, the potential fall area from the
walking/working surface to the net shall be unobstructed
11 Safety nets and safety net installations shall be drop-tested at the jobsite
after initial installation and before being used as a fall protection system,
whenever relocated, after major repair, and at 6-month intervals if left in one
place. The drop-test shall consist of a 400pound (180 kg) bag of sand 30 +
or - 2 inches (76 + or - 5 cm) in diameter dropped into the net from the
highest walking/working surface at which employees are exposed to fall
hazards, but not from less than 42 inches (1.1 m) above that level
12 When work being performed overhead where something could fall or be
dropped, a barricade tape (black & yellow) shall be installed around the
area at least 10 feet from the work zone in order to keep falling objects
within the barricaded area.
Above-mentioned hazard control measures reviewed, resources required for implementing control measures (including PPE) available. By the implementation of above-mentioned
control measures the risk is reduced to acceptable level.
Initial control measures (control measures which are required to be implemented before starting the work) are implemented and subsequent control measures (control measures which
can be implemented at the time of work execution) will be implemented.
