I, the undersigned, Parish Priest of St. Aloysius Gonzaga Parish, request the
permission that the marriage of Mr. Gerard Bryan M. De La Cruz, who was baptized in
your parish on August 22, 1993, domiciled at Sto. Tomas Proper, Baguio City;

and Mary Anne M. Bernadas, baptized in Saint Joseph the Worker Parish,
Pampanga domiciled at Sto.Tomas Proper, Baguio City;

May be licitly celebrated outside their own parish by me or another priest having already
the required faculty to assist validly in my Parish on October 29, 2022.
