On the 26th of July, 2022 at about 9 am, staffs of the Municipal Civil Registrar
of Sablayan namely Jeremiah Magday, Admin Asst, Deson Tapil Admin Asst, Nimrod
Baranda, Admin Asst under the supervision of Municipal Civil Registrar of Sablayan,
Atty. Melvie S. Dacayanan together with the Philippine Statistics Office in the person
of Reymar Rabi, Birth Registration Agent conducted Community Orientation and
Mobile Registration at Barangay Covered Court, Barangay Ligaya, Sablayan,
Occidental Mindoro.

The activity was conducted in compliance with RA 11055 otherwise known as

the "Philippine Identification System Act", which aims to establish a single national
identification system for all citizens and resident aliens of the Republic of the
Philippines. In the conduct of the activity the MCR and PSA received 163 requests for
Omnibus Certification.

Applicants with negative omnibus certification will be assisted in registering

their birth via late registration or out of time registration. All fees accruing to the
clients via PBRAP Project are waived.

identification system for all citizens and resident aliens of the Republic of the
Philippines. In the conduct of the activity the MCR and PSA received 58 requests for
Omnibus Certification.

During the orientation process, clients were informed that the certification is
necessary in lieu of PSA Negative and they will be registered only after the certification
but they don’t have to worry because all fees are waived under PBRAP activities.

of Sablayan, Atty. Melvie S. Dacayanan together with the Philippine Statistics Office
in the person of Reymar Rabi, Birth Registration Agent conducted Community
Orientation and Mobile Registration at Barangay Covered Court, Barangay Ilvita
Sablayan, Occidental Mindoro.

The activity was conducted in compliance with RA 11055 otherwise known as

the "Philippine Identification System Act", which aims to establish a single national
identification system for all citizens and resident aliens of the Republic of the
Philippines. In the conduct of the activity the MCR and PSA received 16 requests for
Omnibus Certification.

Applicants with negative omnibus certification will be assisted in registering

their birth via late registration or out of time registration. All fees accruing to the
clients via PBRAP Project are waived.

The activity ended at 4 o’clock in the afternoon.

Sablayan joined the ELCAC Serbisyo Caravan at Sitio Balani, Barangay Ligaya
Sablayan Occidental Mindoro.

The ELCAC caravan was conducted pursuant to EO 70 or the whole of nation

approach towards ending the armed conflict in insurgency rich areas. The caravan
aims to deliver government services in marginalized communities and geographically
isolated and disadvantaged areas.

Sablayan conducted the second Kasalang Bayan for the year 2022. This Kasalang
Bayan is the first of the same event under the administration of the new Local Chief
Executive.

The event started with the marching of principal sponsors, then the couples
who will get married. In attendance were OIC Administrator Norman Novio, who
gave his opening remarks, Vice Mayor Edwin Mintu, SB Member Alfredo Ventura, Jr,
SB Mark Anthony Legaspi, SB Marfinn B. Dulay, SB Conchita Dimaculangan and
Executive Secretary Zernann Toledo who delivered the speech for Congressman Odie
Tarriela.

Highlights :

On the 10th, October 2022, 2022 at 10 in the morning, the Municipal Civil
Registrar of Sablayan submitted its monthly report for the month of September at
Philipppine Statistics Authority Provincial Office, Mamburao, Occidental Mindoro.

Said transmittal was received by Reymar Rabi, Birth Registration Agent for PSA
PO Mamburao. For the month of September, the MCR Sablayan have
issued/registered 164 Certificate of Live Birth, 7 Certificate of Marriage, 43
Deaths and 56 legal instruments.

For the Month July to September of the Year 2022

I. INTRODUCTION

The primary purpose of civil registration is to establish the legal

documents required by law. Civil registration generates
documentation that supports an individual's right to recognition as a
person before the law and acknowledges their formal relationship with the
state. The civil registry provides individuals with the documentary
evidence required to secure recognition of their legal identity, their family
relationships, their nationality and their ensuing rights, such as to social
protection and inheritance. It can help facilitate access to essential
services, such as health, education, and social welfare and can contribute
to activities such as gaining formal employment, exercising electoral
rights, transferring property, and opening bank accounts. The lack of civil
registration during crisis or natural disasters can lead to statelessness. The
arrival of new technology in resource-constrained environments has
created opportunities for developing more efficient information systems.

Proclamation No. 437 Series of 1994 declared the registration of

births, deaths, marriages and foundlings as a national policy. The Local
Civil Registry Office is the first line of defense in ensuring that this
declaration is being put into action. Therefore it is imperative that the
local government unit, take strict control measures to fulfill the right of the
child to a name, identity and nationality and to protect the Filipino citizens
from birth certificate frauds, the protection of the civil status of and the
record of death of their deceased constituents.

The local civil registry office although under the Local Government
is under the administrative supervision of the Civil Registrar General who
is the administrator of the Philippine Statistics Authority (PSA) and thus
required to submit to the PSA the original duplicate copies of the registred
vital events.

II. HIGHLIGHTS

II.1 Submission of Monthly Reports to the PSA –PO Mamburao.

-On August 5, 2022, the LCRO-Sablayan submitted the duplicate copies of

the recorded vital events from July 2022. For the month of July 2022, the
LCRO-Sablayan have registered 128live births, 17 marriages, 50 deaths

and 45 legal instruments.

-On September 8, 2022 the LCRO- Sablayan submitted its monthly report
for the month of August, 2022 at PSA Provincial Office. For the month of
August, 2022, LCRO- Sablayan recorded 318 live births, 16 marriages, 64
deaths and 78 legal instruments.

-On October 6, 2022 the LCRO-Sablayan submitted its monthly report for
the month of September, 2022 at PSA Provincial Office. For the
month of September, 2022, the LCRO-Sablayan recorded
164 live births, 7 marriages, 43 deaths and 56 legal instruments

II. 2 Conduct of Community Orientation Mobile Registration at

Barangay Ligaya, Sablayan, Occidental Mindoro

On the July 26, 2022, at 8am to 2pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Ligaya,
Sablayan, Occidental Mindoro. The activity was conducted as
part of the PhilID Birth Registration Assistance Project.

II.3 Conduct of Community Orientation Mobile Registration at

Barangay Tagumpay, Sablayan, Occidental Mindoro

On the July 28, 2022 at 8am to 2pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Tagumpay,
Sablayan, Occidental Mindoro. The activity was conducted as part of
the PhilID Birth Registration Assistance Project.

II. 4 Conduct of Community Orientation Mobile Registration at

Barangay Paetan, Sablayan, Occidental Mindoro

On the July 29, 2022 at 8am to 2pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Paetan,
Sablayan, Occidental Mindoro. The activity was conducted as
part of the PhilID Birth Registration Assistance Project.

II. 5 Conduct of Community Orientation Mobile Registration at

Barangay Sto. Nino, Sablayan, Occidental Mindoro

On the August2, 2022 at 8am to 2pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Sto. Nino,
Sablayan, Occidental Mindoro. The activity was conducted as part of
the PhilID Birth Registration Assistance Project.

Barangay San Nicolas , Sablayan, Occidental Mindoro

On the August 4, 2022 at 8am to 2pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at

barangay covered court at Barangay Sto. Nino, Sablayan, Occidental

Mindoro. The activity was conducted as part of the PhilID Birth
Registration Assistance Project.

II.7 Conduct of Community Orientation Mobile Registration at

Barangay Ibud, Sablayan, Occidental Mindoro

On the August 8, 2022 at 8am to 2pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Ibud, Sablayan,
Occidental Mindoro. The activity was conducted as part of the
PhilID Birth Registration Assistance Project.

Barangay Buenavista, Sablayan, Occidental Mindoro

On the August 11, 2022 at 8am to 2pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Buenavista,
Sablayan, Occidental Mindoro. The activity was conducted as part of
the PhilID Birth Registration Assistance Project.

On the August 15, 2022 at 8am to 12 pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Malisbong
Sablayan, Occidental Mindoro. The activity was conducted as part of
the PhilID Birth Registration Assistance Project.

On the August 15, 2022 at 8am to 5 pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile

On the August 16, 2022 at 8am to 5 pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Tuban
Sablayan, Occidental

Mindoro. The activity was conducted as part of the PhilID Birth

Registration Assistance Project.

Barangay Ilvita, Sablayan, Occidental Mindoro

On the August 16, 2022 at 8am to 5 pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Ilvita, Sablayan,
Occidental Mindoro. The activity was conducted as part of the
PhilID Birth Registration Assistance Project.

On the August 17, 2022 at 8am to 5 pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Claudio Salgado,
Sablayan, Occidental Mindoro. The activity was conducted as
part of the PhilID Birth Registration Assistance Project.

II.14 Conduct of Community Orientation Mobile Registration at

Barangay Sta. Lucia Sablayan, Occidental Mindoro

On the August 18, 2022 at 8am to 5 pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Sta. Lucia ,
Sablayan, Occidental Mindoro. The activity was
conducted as part of the PhilID Birth Registration Assistance
Project.

On the August 22, 2022 at 8am to 5 pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Burgos,
Sablayan, Occidental Mindoro. The activity was
conducted as part of the PhilID Birth Registration Assistance

II.16 Conduct of Community Orientation Mobile Registration at
Barangay GEA Sablayan, Occidental Mindoro

On the August 23, 2022 at 8am to 5 pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay GEA, Sablayan,
Occidental Mindoro. The activity was
conducted as part of the PhilID Birth Registration Assistance
Project.

On the August 30, 2022 at 8am to 5 pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay San Vicente,

conducted as part of the PhilID Birth Registration Assistance
Project.

II.18 Conduct of Community Orientation Mobile Registration at

Barangay San Agustin, Sablayan, Occidental Mindoro

On the August 31, 2022 at 8am to 5 pm the LCRO and LCRO staff of
Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay San Agustin,
Sablayan, Occidental Mindoro. The activity was
conducted as part of the PhilID Birth Registration Assistance Project.

II.19 Conduct of Community Orientation Mobile Registration at

Barangay Pag-Asa, Sablayan, Occidental Mindoro

On the September 1, 2022 at 8am to 5 pm the LCRO and LCRO staff of

Sablayan conducted Community Orientation and Mobile
Registration at barangay covered court at Barangay Pag-Asa,
Sablayan, Occidental Mindoro. The activity was
conducted as part of the PhilID Birth Registration Assistance Project.

II.20 Conduct of ELCAC Caravan at Barangay Ligaya,

Sablayan, Occidental Mindoro

On the September 15, 2022 at 8am to 5 pm joined the ELCAC Caravan at

Sitio Balani, Barangay Ligaya, Sablayan, Occidental Mindoro.

II.21 Conduct of Kasalang Bayan 2022

On September 30,2022 the MCR conducted Kasalang Bayan, September

30, 2022 on Sablayan Convention Center, participated by 50
couples.

Highlights :

On the 25th of August, 2022 at 9:00 am the MCR of Sablayan represented by

Ms. MindaDeOro De Jesus, Admin Asst IV participated in the Civil Registration
Training for Medical Personnel. In the said training, Ms Minda De Jesus discussed the
rules on how to fill up the entries in the Certificate of Live Birth.

Also in attendance are the different Civil Registrars and MHOs as well as
representatives from different hospitals. The activity ended with photo ops together
with PSA Regional Director Leni Rioflorido.

On the 26th of August 2022, the Municipal Civil Registrar of Sablayan,

represented by MindaDeOro De Jesus, Admin Asst IV attended the launching of the
first PhiSys, fixed registration center located at Baby A Space Rental, Niebres Street,
Barangay 7, Mamburao, Occidental Mindoro.

In attendance are representatives from different Municipal Civil Registrars

from Occidental Mindoro. Also in Attendance is PSA Regional Director Leni Rioflorido
