PROCEED WITH THE ONLINE APPLICATION.

VISIT THE LINK oad.clsu2.edu.ph/user-login


and click APPLICANT (CLSU College Admission Test)

STEP 1

A Fill in your LAST NAME, FIRST NAME AND MIDDLE NAME.

B Please provide your current personal Gmail address and request an OTP (One
Time Password). Enter the 6-digit OTP that has been sent to your email. You
may check the SPAM folder if not found in your inbox.
C Indicate your date of birth.

D Create and confirm your password (at least 8 characters). Remember this
password, as you will use it every time you sign in to the portal. Please note
that this password is not the same as the one for your Gmail account
E Click submit to complete the process.

STEP 2

Make sure to supply all required information. There are 4 parts to be completed
which include:
A Category of Applicant
B Course Choice
C Personal Information*
D Educational Background

*NOTE: In this part, you will be asked to upload an electronic copy of your recent
picture (2x2 colored, studio-taken, white background, with nametag: SURNAME,
FIRST NAME, MIDDLE INITIAL, file size less than 2MB)

REMINDER
Please review all information. before clicking the submit button.
Once submitted, you cannot edit the application form.
STEP 3

These include:
A Scanned copy of PSA/NSO birth certificate;
Scanned copy of SF-10 or Form 137-A (Junior High School, Grade 7-10),
B
reflecting the grade point average (GPA) in each grade level, and duly
signed by the school principal or school registrar*;
Scanned copy of SF-9 or Form 138 (Grade 11), reflecting the grade point
C
average (GPA) in each semester, and duly signed by the school principal
or school registrar*
*NOTE: For Alternative Learning System (ALS) graduates - Certificate of
Rating/Equivalency issued by the Bureau of Education Assessment.

REMINDER
It is recommended that you .name the file appropriately to avoid
incorrect/improper uploading.

Scanned Copy of SF-9/Form 138 YOURSURNAME_Form138.pdf

*NOTE: If the document is more than 2 pages, merge them into a single
PDF file.

STEP 4

Make sure to supply all information.

STEP 5

After printing the application form, AFFIX YOUR SIGNATURE ABOVE YOUR NAME.
Ask your parent/ guardian to do the same. Do not forget to indicate the date. The
test permit includes information on the venue, day, and time of the examination. It
also includes important reminders, so read it carefully.

You will bring the ❶ signed application form, ❷ test permit, ❸ school ID or any
valid ID with picture, and ❹ photocopies of all documentary requirements during
the examination day.
