Dear Ms. Riddhiben Joshi,
‘Sub: Your Appointment for the post of Staff Nurse:
Ref: Interview you had with us
We are pleased to inform you that you are hereby appointed as Staff Nurse at Global Long Life Hospital
& Research Pvt Ltd with effect from 01/12/2018 subject to the following terms and conditions:-
1. You will be on probation for a period of 6 months from the date of your joining this Company as
Staff Nurse to enable the Management to assess your efficiency and fitness, The said probation
Period can be extended further for the maximum period of 6 months in case the same is
considered necessary by the Management. On your work and behavior being considered
satisfactory by the Management, your appointment will be confirmed in writing.
2. Until and unless your services are confirmed in writing, you will be deemed to continue as an
employee on probation irrespective of the department to which you may belong or the job you
may carry on at that time.
3. Your salary will be Rs. 14,000/- pm
In case, the company has to pay Deamess Allowance or any other allowances arising out of
statutory requirements, the same wil be adjusted in the above mentioned salary.
4. You may be considered for increment as per the rules of the Company and the same is purely
dependent on your job performance, achievements, behavior, attitude, timely reporting, honesty,
integrity and other merits. Management decision on this shall be final & binding on you and shall
not be consider merely as amatter of right.
5. a) The notice period during probation in case of leaving services shall be with prior 15 days and
confirmation with prior 30 days’ notice without adjusting leaves, if any. Gross salary will be
deducted [Notice Pay] for not serving the above mention notice period
b) Leaving services must be tum acceptance of resignation & handing over charge to your
HoD/Director and HRD falling to which full & final settlement shall not be processed company.
6. The details of your job responsibilities will be intimated to you after joining the services.
7. Your appointment will be subject to verification of your service record and antecedence. In case
any information furnished by you in connection with the above appointment found incorrect at any
stage o correct information is found suppressed, your services are liable to be terminated at any
time without any notice.
8. If any tme in our opinion which is final in this matter, you are found guilty of dishonesty,
disobedience, disorderly behavior, negligence, indiscipline, absence from duty without permission
or any other conduct considered by us detrimental to company's interest or of violation of one or
more terms of this letter, your services may be terminated without any notice.
9. During the tenure of your services with the Company, you will not be allowed to engage yourself
in any other work or carry out any other assignment which interferes with or affects the efficiency

10. Your services are transferable anywhere in the country at the sole discretion of management
without change in your salary structure prevailing at the time of transfer.
11. You shall maintain utmost secrecy in respect of all matters, reports, operation, processing,
inventions etc, with which you may come in contact during your employment with the company.
and you shall not disclose any information regarding the affairs of the company which come to
your knowledge during the period of your services, the disclosure of which information is
prejudicial to the interest of the company.
12. You will be responsible for safekeeping end submission of all company's property in good
coneition, which may be in your use, custody or charge.
13. You will keep the Company informed of your exact address and will advise immediately the
change, if any, in the same. In the absence of your latest / current postal address in the record
of the Company, any communication dispatched to you at the address mentioned in your
prescribed application form / appointment letter address shall hold good and you will not have
dispute for the same.
14, You shall retire on attaining the age of 58 years and shall not have any claim to be continued in
service thereafter.
15. The company is obliged to deduct Income Tax at source as per provision of Income Tax Act /
Rules /. Accordingly, you are required to submit all required proof of permitted savings /
investments and other details from time to time to enable the company to comply with the
provisions of law. In the event of noncompliance by you as aforesaid if the company is required
to pay any interest or payment under Income Tax Act, it shall deduct the amount as may be paid
or payable from your salary or other payments and you haill allow the company to comply with
these requirements without objection.
16. This appointment letter is issued subject to Ahmedabad Jurisdiction only
