PARENTAL CONSENT

I am giving my full consent as a parent/legal guardian to my son/daughter,

___________________________, for a Tree planting activity at Sitio San Ysidro, Brgy. San Jose,
(Name of Student)

Antipolo City. I understand that this activity is part of the course NSTP 1 National Service
Training Program which is a requirement to complete the academic degree for which my
son/daughter is enrolled in. To the extent that the obligations of the school in safeguarding the
personal welfare of student is complied with, I hereby hold Jose Rizal University including its
Board, Management, officials and Employees free from any and all liabilities arising from any
loss, damage or injury that may occur during the period of the internship and while the student is
actively engaged in the undertaking.

The Faculty and the Chairperson will be visible at all times during activity.
