Dear Sir,
We are pleased to submit our post general pest control treatment activities report
conducted in your facilities DOLE Main Building, Annex and Executive bldg. and BF
Condominium for month of December 02, 09, 16,23, & 29, 2023.
PEST CONTROL ACTIVITIES:
DOLE MAIN BUILDING & ANNEX AND EXECUTIVE BUILDING & BF
CONDOMINIUM
Presence of German cockroaches was observed during treatment in
following areas:
Low infestation observed at main bldg. ground floor pantry.
Mosquitos’ observation during treatment was noticed as follows:
Moderate infestation at ground floor lobby and security office
due to open doors and windows.
Heavy infestation at Main bldg. perimeter due to open area.
Heavy infestation at annex & executive bldg ground floor and
perimeter area and pile stock on the side.
Low infestation observed in BF Condominium.
OTHERS
Deployed five (5) personnel during treatment activities
assisted by authorized personnel.
Chemical used was CYMFLEX, ATTRATHOR,OPTIGARD
ROACH GEL BAIT and KLERAT for rat bait and Bosny rat
glue for customized rat traps.
All concern areas were intensively treated.
MAC J Team performed manual spraying, ULV /Misting
treatment, and rodent control.

All non-active termite’s stations are indication that baiting system
installed are working and effective.

Rodent Control Program:
Placed of rodent traps and rodent bait strategically were rats reported
positioned in the building. Attached rodent forms including monitoring
report form.
Disposed klerat at Annex & executive bldg. perimeter area.
