I, ROSELYN M. KITAY, Filipino citizen, of legal age, single, and a
resident of Dumaguete City, Negros Oriental, after having been sworn in
accordance with law, state and declare:
1 – That I am the owner of a TM Sim Card with Contact No.
09676938398;
2 - That I lost my TM Sim Card last November 14, 2023 together with
my cellphone;
3 - That effort was made to retrieve the same, however, the same proved
futile.
4 - That I am executing this affidavit to prove the loss of the TM Sim
Card in support of my application with the Globe Telecom for a replacement;

INFORMATION
THE STATE, thru the undersigned Assistant Provincial Prosecutor of
the Province of Negros Oriental accuses GENELOU MACAJIG a.k.a
“GM”, of the crime of violation of Section 11, Art. II, of RA 9165 committed
as follows:
That on November 14, 2023, at 2:52 PM at Barangay Taclobo,
Municipality of Dumaguete, Province of Negros Oriental, Philippines,
and within the jurisdiction of this Honorable Court, the above-named
accused, not being lawfully authorized by law, did then and there
willfully, unlawfully and knowingly have in his possession and control
four (6) pieces of heat-sealed transparent plastic sachets containing
Methamphetamine Hydrochloride or "shabu", with a total net weight
of sixty-nine point six hundred sixty-three (69.663) grams, a
dangerous drug.
