AFFIDAVIT OF LOSS
I, ROSEMARIE R. BIGTAS, of legal age, widow, Filipino Citizen, with residence and
postal address at Purok 2, 26A, Irisan, Baguio City, Philippines, after having been duly sworn to
in accordance with law do hereby deposes and states, that:

1. I am engaged in business particularly a certain stall, Stall No. 23 Sari-Sari Extension,


Hanger, City Public Market, Baguio City, under the name of my late husband
Orlando M. Aoanan;

2. The said stall was recently awarded in my favor;

3. The said business or stall was duly registered with the Bureau of Internal Revenue
(BIR) and was issued a CERTIFICATE OF REGISTRATION, ASK FOR
RECEIPT (Red Color) and OFFICIAL RECEIPTS (OR);

4. Unfortunately though, the said documents were lost, including the unused receipts. I
am not sure how these documents were lost, maybe it was misplaced or accidentally
thrown away;

5. I exerted diligent effort to find the said documents, however, I failed, hence, lost
forever and beyond recovery;

6. The said CERTIFICATE OF REGISTRATION, ASK FOR RECEIPT (old) and


unused OFFICIAL RECEIPTS (OR) have not been given nor pledged to anyone
for any purpose whatsoever, nor were taken nor confiscated by the authorities;

7. I am executing this affidavit to attest to the truth of my foregoing statements and for
all legal intents and purposes it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 15th day of January, 2024,
at Baguio City, Philippines.
