Estate, Alfonso Cavite, after having been sworn to in accordance with law, hereby depose
and state:
1. That my name is REGINE SOLITA, born on May 15, 2003 in Pambuan, Gapan
Nueva Ecija, daughter of Estelita V. Solita;
2. That I am a citizen of the Philippines;
3. That I am presently applying for the delayed registration of my birth with the Office
of the Local Civil Registrar of Gapan Nueva Ecija;
4. And one of the requirements is my school records, but I only completed
kindergarten when I was in school, and as such, that is my only school record.
5. I am executing this Affidavit to affirm the truthfulness of the foregoing and for
whatever legal purpose this may serve;
