AFFIDAVIT OF LOSS
I Jay Ann C Geroy, of legal age, Filipino and a resident of Brgy. Cambante
Mercedes Eastern Samar, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and say that:
1. That I am the owner of the Professional Regulatory Commission ID (PRC),
Philippine National Police identification card, Driver’s License ID, Pag-Ibig ID,
personal ATM card, and TIN ID;
2. That I have usually kept the same identification cards inside my wallet and
brings with me wherever I go;
3. That few weeks ago, I went to Borongan, City to buy some stuffs however
upon arrival at my destination, I noticed that I lost my wallet was missing
including all my important ID’s and ATM card;
4. That I tried to look for the same traversing back my path wherein I could
possibly dropped my wallet and asked some people along the way but despite
my efforts of finding the wallet, my search for it and my hopes of finding the
same proved futile;
5. That because of this, I declared the same to be lost and cannot be found; and
6. I am executing this Affidavit in order to attest to the truth of the foregoing facts
and for other legal purpose(s) this may serve best.

FIRST PARTY/SELLER hereby unconditionally and absolutely sell, convey
and transfer, free from all liens and encumbrances unto its successors and
assigns, the aforementioned One Thousand Three Hundred Six (1,306)
Square Meters parcel of land and the SECOND PARTY/BUYER hereby
buys the same from the FIRST PARTY/SELLER subject to the following
ters and conditions, to wit:
1. The principal and total consideration in the amount of FOUR
HUNDRED THOUSAND (P400,000), Philippine Currency, is to be paid in
full upon execution of agreement, which receipt thereof is hereby
acknowledged upon signing of this deed;
2. That the FIRST PARTY/SELLER shall be responsible for the payment of
Acknowledgement and Notarial Fees before a Notary Public; and
3. The SECOND PARTY/ BUYER shall be responsible for Capital Gains
Taxes, Documentary Stamp Taxes and all other Tranfer Taxes insofar aas
the subject parcel is concerned.

Philippines, having been born on the 11 th day of September 1994, now
actually residing at Brgy. Cambante, Mercedes Eastern Samar being of
sound and disposing mind and memory, and not acting under undue
influence, violence, fraud, or intimidation of whatever kind, do by these
presents declare this to my Last Will and Testament which I have caused to
be written in English, the language which is known to me. And I hereby
declare that:
1. I desire that my remains be buried according to the rites of the
Roman Catholic Church and with dignity suitable to my
circumstances;
2. I give and bequeath to my son, Prince Jeaxiann Caden C.

Shipping Lines, 1 land rover in red color, 1 audi r8 couple in
white color, a mansion at Forbes Park, Makati City with 1800
square meters, a real property with lot no. 1234-A located at
Brgy Banahao, Guiuan Eastern Samar, 4321-A located at Brgy
San Jose Mercedes Eastern Samar, 5678-A and Brgy
Naparaan Salcedo Eastern Samar, a cash at BDO-Guiuan
Eastern Samar amounting to 600 million;
3. I give and bequeath to my sister, Andres Lizz A. Dela Rosa, all
my real property located at Brgy Alingarog with lot no. 12345-B,
lot no. 56789-B located at Brgy Sung-an Mercedes Eastern
Samar, and a residential house and lot Brgy 1 Pob., Mercedes
Eastern Samar, and a cash at BDO-Guiuan Eastern Samar with
400 million cash;
4. I designated Maria Lilibeth A Rasona, as the sole executor
and administrator of this Last Will and Testament, and in her
incapacity, I name and designate Alyssa Francine G Valdez as
her substitute.
5. I hereby direct that the executor and administrator of this Last
Will and Testament or her substitute need not present any
bond;
6. I hereby revoke, set aside and annul any and all of my other will
or testamentary dispositions that I have made, executed,
signed or published preceding this Last Will and Testament.
