PETITION

PETITIONER, unto this Honorable Registry of Deeds for the City of Davao, most
respectfully states:

1. That petitioner, is of legal age, Filipino and resident of Matina, Davao City,
Philippines;

2. That petitioner is the vendee of two (2) parcel of land situated in Brgy. Sirib, Calinan,
Davao City, covered by Transfer Certificate of Title Nos. TCT 146-2021015125
and TCT 146-2021015126 of the Registry of Deeds for the City of Davao, containing
an area of One Thousand Seven Hundred Five (1,705) Square Meters and One
Thousand Seven Hundred Eighty-Seven (1,787) Square Meters, respectively.
Copies of the titles of the said properties are hereto attached for ready reference:

3. That petitioner intends to consolidate the said properties and subdivided the same in
to seven (7) lots;

4. That petitioner engaged the professional services of Engr. Raymund D. Cabrella, a


dully licensed Geodetic Engineer to subdivide the said property and identified as Lot
9997-A-1 and Lot 9997-A-2, PSD-11-151101 inclusive, approved and certified as
simple in the Plan No. PCS-11-007212 was duly approved by the Director of Lands,
Region XI, Davao City on March 15, 2022;

WHEREOF, it is most respectfully prayed of this Honorable Register of Deeds for the
City of Davao to totally CANCEL TCT NO. 146-2021015125 AND TCT NO. 146-2021015126
and in lieu thereof issue a new title and separate titles of sub-lots hereunder identified making
the basis for the issuance of the approved plan, advanced technical descriptions and other
documents attached thereto, consisting of the seven (7) lots, which will be allocated to its
members, to wit:
