I hereby willingly and voluntarily give consent to the participation of my son

RC Farm, San Vicente, Makilala, Cotabato on March 15, 2024, Friday, 7:30 PM.

I am delighted to show my full support, as my son’s participation in this basketball


cup will bring honor and pride to our barangay, provided that due care and precaution
will be observed to ensure the safety of my son. The Sangguniang Kabataan Council of
Barangay Kisante may not be held responsible for any untoward incident that may
happen beyond their control.
