Madam,

Greetings!
May I request that my PPSTA deduction (0044A PPSTA P253, and 0044C
PPSTA P180 a total of Php 433) be removed and stopped on my monthly payroll/
salary. I have already submitted the requirements in the PPSTA office since
December 22, 2022 and attended the PPSTA Committee Exit Seminar last
December 7, 2022 but until now it still reflects on my monthly pay slip. It is important
for me to remove and stop its deduction to my monthly payroll so that I can finally
submit the last requirement needed for refund.
Attached is my pay slip for the month of February and March and a letter of
confirmation from the PPSTA Claim Department during the Committee Exit Seminar
I attended for your reference.
I am hoping for your favorable action to this matter. Thank you very much.
