of State personnel have reviewed the application.
By clicking “Sign and Submit Application” you are electronically signing the application. You are required to
electronically sign your application yourself, unless otherwise exempt by regulation, even if the application
has been prepared by someone other than yourself. Your electronic signature certifies that you have read

Additionally, by clicking “Sign and Submit Application” you are certifying that you understand that you are

understand that any willfully false or misleading statement or willful concealment of a material fact made by
you within the application may subject you to permanent exclusion from the United States and, if you are

visa eligibility consistent with INA Sections 212(a) and 221(d), and will be notified of the requirement. If you
are notified and required to undergo a medical examination, failure to provide required information may
cause delay or denial of your visa application. If required to undergo a medical examination, your medical
examination information may be collected and temporarily stored in the eMedical system hosted, operated,
and maintained by the Australian Department of Home Affairs. If your medical examination is collected in
eMedical, you will be requested to provide consent to its collection and temporary storage in such system,
and being transferred to the U.S. Government for the purposes of enabling the U.S. Department of State to
determine your medical eligibility and for the U.S. Centers for Disease Control and Prevention to undertake
public health functions under the Public Health Service Act Section 325 and INA Section 212(a).

Chinese citizens with 10-year B1, B2 or B1/B2 visas in Peoples’
Republic of China passports will be required to update their
biographical and other information from their visa application via a
website every two years, or upon getting a new passport or B1, B2, or
B1/B2 visa, whichever occurs first. This mechanism is called EVUS -
Electronic Visa Update System. The EVUS website is now open to the
public for enrollments at www.EVUS.gov . CBP will not collect a fee
for EVUS enrollment at this time. CBP anticipates the eventual
implementation of an EVUS enrollment fee, but does not have a time
frame. Until the implementation of a fee, travelers can enroll in EVUS
without charge. The Department of Homeland Security, Customs and
Border Protection (CBP) will keep visa holders informed of new
information throughout the year. For further information, please
visit www.cbp.gov/EVUS .
Attention: Before you enter the receipt number ensure that the passport bio data
and DS-160 information matches the ustraveldocs profile for all applicants

after they are assigned to a particular applicant.

process your visa application if your DS-160 barcode number is different from the
number on your Appointment Confirmation letter. This will cause delay in applying
for and receiving your visa. To avoid delay, please ensure the following:
1: Make sure the DS-160 barcode number matches the barcode number in your
UStraveldocs profile and on your Appointment Confirmation letter.
2: If you applied for a U.S. visa previously, please update your current DS-160
barcode number in your UStraveldocs profile.
3: We suggest you copy/paste the DS-160 barcode number from the electronic

information may require completion of a new profile and purchase of a new MRV fee receipt. MRV fees are not

5. If you have encountered an error with your payment or you believe your payment was not successful, DO NOT PAY
THE FEE A SECOND TIME AS IT WILL NOT BE REFUNDED. We recommend you check your bank account. If your

change. CGI is neither responsible for determining these criteria nor able to guarantee any appointment or other services.
7. The estimated wait time to receive an interview appointment at a U.S. Embassy or Consulate can change and is based
on actual incoming workload and staffing. These are estimates only and do not guarantee the availability of an
