AFFIDAVIT OF LOSS

I, Al Vincent Ma G, Delgado, of legal age, Filipino, 3390 Magistrado


Arellano St. Bacood, Sta. Mesa, Manila, after having been sworn in
accordance with law, hereby deposes and states the following THAT:

1. I am the owner of a motor vehicle specifically described as follows1:

Make : Honda Civic


Plate No. : PRQ 941
Engine No. : R18A17704600
Chassis No. : PADFD1640AV412166

2. Said vehicle was issued the Plate No. PRQ 941 by the Land
Transportation Office of ****;
3. That said vehicle was also issued an Official Receipt (OR) and
Certificate of Registration (CR).
4. Unfortunately, the said plate was stolen from the motor vehicle
sometime in 2023 at Sta. Mesa, Manila, and it was only the next day,
that I noticed that the plate number of the vehicle was gone from its
place;
5. The Car Plate Number was never lent to another person or entity nor
confiscated by any other law enforcement agency;
6. Due efforts has been made in order to retrieve said Car Plate Number
but to no avail;
7. The aforesaid car plate number is irretrievably lost and can no longer
be found despite diligent efforts to locate the same;
8. I bind myself to surrender said Plate to the Land Transportation Office
that issued the same in the event that said plate is recovered
9. That I am executing this affidavit to attest to the foregoing facts and
for legal purpose and intent this may serve..
