Canadian Career Education College does not guarantee


employment for any student who successfully completes a
vocational program offered by Canadian Career Education College.
It is understood that fees are payable in accordance with the fees
specified in this Enrolment Contract and all payments of fees shall
become due forthwith upon a statement of accounting being rendered.

Enrolment Contract if the undersigned student does not attend classes
during the first 14 days of the program after it begins. For information
regarding cancellation of this Enrolment Contract and refunds of

Society for Medical Laboratory Science (CSMLS) or the Medical Laboratory
Professionals' Association of Ontario (MLPAO). I understand that if I want to
challenge the CSMLS/MLPAO certification, I will be required to complete a Prior
Learning Assessment and Recognition (PLAR). I further understand that the PLAR
process does not guarantee my ability to challenge the certification

Notice of Collection of Personal Information and Consent (Ontario

Ministry of Advanced Education and Skills Development) Domestic

Students or International

Career colleges must be registered under the Ontario Career Colleges


Act, 2005, which is administered by the Superintendent of Career
Colleges. The Act protects students by requiring career colleges to follow
specific rules on, for example, fee refunds, training completions if the
college closes, qualifications of instructors, access to transcripts and
advertising. It also requires colleges to publish and meet certain
performance objectives that may be required by the Superintendent for
their vocational programs. This information may be used by other
students when they are deciding where to obtain their training. The
consent set out below will help the Superintendent to ensure that current
and future students receive the protection provided by the Act.

address, telephone number, e-mail address and other contact
information to the Superintendent of Private Career Colleges for the
purposes checked below:

objectives required by the Superintendent for its vocational

that if I refuse or withdraw my consent the Superintendent may not be
able to contact me to inform me of my rights under the Act or collect

Schedule "A"
Tuition Payment Plan Agreement and Schedule
This document is to confirm the arrangement under which Can-College
will accept payment of your total tuition fee. Can-College has approved
your desired payment plan according to the following agreed schedule/
terms:
Total Tution Fee: 7400.00
**Monthly Payment Schedule (No. Of Months): 6
**Advance Tuition fee: 1233.33
Payment Start Date: July 10, 2024
Payment End Date: December 10, 2024
**Note. This amount will be deducted from the total tuition amount and
the remaining balance will be paid according to the selected monthly
payment plan.
The student (Rosemond Opoku Agyemang) agrees to this payment
plan as prescribed for the student’s outstanding account balance. If the
student fails to response or is delinquent in payment the college
reserves the right to take necessary measures.
The student listed above has agreed to the above payment schedule
and understand that the diploma if graduated successfully will be
awarded upon final payment according to the schedule.

PCC Responsibilities Related to


Distance Education Program Delivery
Adendum DE - Can-College- Apendix B
This addendum is part of the Student Enrollment Contracts relating
to DE programs and is subject to the Private Career Colleges Act,
2005 and the regulations made under the Act.
I Rosemond Opoku Agyemang am enrolling in the Personal Support and
Child Care Diploma at Canadian Career Education College LTD. I
understand that this program will be delivered using a Distance
Education mode of delivery. This means that at least a portion of my
training will be delivered using correspondence, audio, video, and/or
computer-based technologies or any combination thereof.I understand
that this PCC will provide me with information about the following
matters:
1. The school’s responsibilities to me as a student enrolled in a
distance education program; Details on how the instruction will be
delivered;
2. A program timetable for each course showing in class time versus
distance education time;
3. Orientation materials (including resources, policies and processes)
on the use of the relevant technologies needed to enable me to
complete my program successfully;
4. Itemized list of all learning materials required prior to the start of
the program;
5. Whether I can use software and/or hardware and/or other
equipment that I currently own or whether I have to buy new
equipment or whether the school will lend me the equipment;
. The technical standards this equipment must meet (e.g. minimum
Internet speed);
7. The Technical support related Q & A links and information has
been provided by the PCC
. If the school lends me the equipment, the duration of the loan and
any fees associated with the loan;
9. Description of the hardware, software and equipment the degree of
technical competence I need in using them;
10. Identification of minimum Internet speed access required;
11. Service Standards of the PCC in relation to this DE program,
including response times to questions regarding assignment
evaluation, grades, technical issues and financial matters;
12. Instructor availability; and
13. The complete, itemized costs of this DE program.

TO CONFIRM YOUR SEAT AT C.C.E. COLLEGE:

1. You must pay your tuition fee amount as listed above and as per agreed
terms in enrollment contract, or 30 days prior to start date of the program or
immediately after receiving study visa-permit approval from IRCC, whichever
comes first. You must have a valid admission or LOA to pay the fee.

2. Your tuition payment will hold your seat in the program and confirms your
commitment to start your studies in the delivery mode that is available (e.g.
on-campus, remote delivery online, or a combination of on-campus and
remote online learning). If the tuition payment is not received as per schedule
mentioned above, your Letter of Acceptance will be revoked.
