Dear Mr. and Mrs. Thorpe,
We would like to take this time to thank you for choosing Justice Tax for all your tax matters. We hope you
found our time together was nothing short of outstanding. We truly understand how stressful this situation
was and glad we were here to help. The Installment Agreement with the Internal Revenue Service was
established and are outlined below:
Your agreement was set up for the 2015. 2018 and 2020 Form 1040 US Individual Income Tax
Return liabilities.
The amount owed is approximately $55,963.84.
The Installment Agreement payments are in the amount of $985.00 per month beginning April 28,
2024.
There is a one-time setup fee in the amount of $225.00 and will be deducted from your first
payment.
The Internal Revenue Service mailed you a confirmation letter to let you know your agreement has
been established along with a voucher to mail in your payment- Please note you may not get
vouchers every month. Your payment will still need to be made.
John and Viorica you requested your payments to be auto drafted from your bank account. I have attached
Form 433D and I do need you to write in the banking information, initial, sign and date the form. Please
return it to my office by April 5, 2024. Once I receive it back, I will forward it to the Internal Revenue
Service. You will need to mail in your first few payments, as it can take upwards of 90 days for the Form
433D to be processed. The IRS will send you a letter letting you know it was accepted and the date the auto
draft will begin, Again, mail in payments until you receive the letter.
Options for making payments:
• By Mail: Payments made by check, money order, or cashier’s check can be mailed to the IRS at
the address listed below and should be sent at least 15 days prior to the due date by certified mail.
The check should be made out to the “United States Department of Treasury” and include on memo
line the phrase “Installment Agreement” along with your Social Security number and daytime
telephone. Also write Form 1040 2015. 2018 and 2020.
• Address to submit payment by mail if you do not receive the voucher.

• Online: you may make a payment through http://www.irs.gov/Payments/Direct-Pay. You have the
option to pay through checking and savings or credit and debit card.
Things to keep in Mind.
Any future refunds will be applied to your prior year balances due to the IRS.
In addition, any payments made above the required minimum can be made without any additional
penalties and will pay off the balance sooner.
Penalties and Interest will continue to accrue at a reduced rate until the balance is paid in full or the
Statute of Limitations to collect has expired.
Compliance Requirement: You are advised to change your withholdings or to make estimated tax
payments, so you do not continue to owe in the future. If you owe additional balances, you will
need to be pay the balance in full by the filing deadline or this could cause this agreement to go
into default. All tax returns must be filed on time moving forward.
In addition, late or missed payments may also result in a default of the agreement.
The IRS reserves the right to enforcement actions if the agreement if not honored which may
include but not limited to levies against your bank account, income, or seizure of property.
Adjustment to Agreement: we understand that changes in life may prevent you from keeping up
with this agreement. We recommend that you DO NOT DEFAULT the agreement; however,
contact our office at (888) 967-7797 so that we can discuss your options.
Again, it has been our honor to be of service to you here at Justice Tax, so you can put this behind you. We
appreciate being a part of the solution for your resolution. Please remember we are always here if you have
any questions or need additional assistance in the future.
Sincerely,
Tammy Alberts, E.A.
