JOINT AFFIDAVIT
We, MARY B. MASACOTE, married; MERLY B.ESPADA, married, and FELY B. ESPADA,
represented by her daughter, of legal age, Filipinos, and residents of Highway,
Poblacion, Alegria, Cebu, afterhaving been duly sworn to according to law, do
hereby depose and say that:

1. We personally know MARIA TERESA VILLARUBIA COMON-SAD who is our close


friend, neighbor and the daughter of thedeceased spouses Abundio
Villarubia Sr. and Purita Gomez Vil-larubia, residents of Municipality of
Algeria;

2. We came to know just lately that both her SSS and PAG-IBIG records input
an erroneous information regarding her name,her SSS and PAG-IBIG
records misspelled the first name as THERE-SA (reflected in her SSS records)
instead of MARIA THERESA (as re-flected in her NSO Birth Certificate);

3. We are executing this affidavit to attest to the fact thatthe names THERESA
(as reflected in her SSS and PAG-IBIG records),and MARIA THERESA (as
indicated in her NSO Birth Certificate) allrefer to ONE and the SAME person

4. In executing this affidavit, we are fully aware and conscious of our duty and
obligation to state only such facts as aretrue and correct of our own
knowledge, as well as of the legalsanction for swearing to any falsehood
under oath; and

5. We hereby affirm under pain of perjury that our forego-ing statements are
all true and correct.
