AFFIDAVIT

I, GRACE KATHRYN E. GENIL, of legal age, single, Filipino, a residence of No. 3173
Anos, Los Banos, Laguna, under oath, depose and state, That:

1. I am a direct hire Overseas Filipino Worker applicant and presently applying as


housekeeper in the Bahamas;

2. I am presently processing my overseas application papers with the different


government agencies particularly the Philippine Overseas Employment Agency
(POEA);

3. Initially, I went to the main office of the POEA located in EDSA, Ortigas, Metro
Manila, to submit the necessary documents needed for my overseas application;

4. However, there are a lot of applicants on queue and it would take days before I
could be accommodated by the POEA;

5. My scheduled flight is on September 10, 2011. The processing of my application


papers at the POEA main Office would delay my intended departure;

6. I was advised by my relatives to go up to Baguio City to process my papers at the


POEA Cordillera Administrative Region because there are less applicants and I
could immediately be accommodated;

7. As such, I am now submitting my complete application documents at the POEA


Cordillera Administrative Region; and

8. I am executing this affidavit to attest to the truth of the foregoing facts without any
intent to circumvent any existing laws and regulations and to comply with the
requirements of the POEA and for all legal intents and purposes which this
affidavit may serve.

in Baguio City, Philippines.

SUBSCRIBED and SWORN to before me, a Notary Public of the City of Baguio this
7th day of September 2011, affiant is personally known to me and identified by me based on
her DFA Passport with I.D No. XX3250383 issued on Mar. 16, 2009 at DFA Lucena City to be
the same person who executed the foregoing instrument, signed the foregoing instrument in
my presence which she acknowledged before me as her free and voluntary act and deed.
