Subject: Show Cause Notice for Unauthorized Absences
Dear Mr. Mondal
With utter dismay, we are writing to address a matter of significant concern regarding your recent
pattern of taking abrupt and unapproved leaves from your position as Asst. Manager-QC in the
Quality Control Department.
It has come to our attention that you have been absent from work on 11-11-23, 13-11-23 & 14-11-23
without obtaining prior approval or providing any explanation for your absence to any one of your
reporting head. Your unauthorized leaves have resulted in a disruption of workflow within the
department and have put an undue burden on your colleagues who have had to cover for your
responsibilities which is absolutely unbecoming and it’s a sheer violation of company discipline.
We understand that personal circumstances may arise that require time away from work, and we are
committed to supporting our employees in such situations. However, it is imperative that you follow
the proper channels for requesting leave and communicate any challenges you may be facing to the
relevant authorities.
As an employee of Mortex India it is your responsibility to adhere to the company's policies and
procedures, including those related to attendance and leave. Unauthorized absences not only violate
company policies but also impact the overall productivity and efficiency of the team and at the same
time you have exhibited with subversive of discipline.
In light of the above, you are hereby required to provide a written explanation for your unauthorized
absences within 24 hours from the date of receipt of this letter. Your response should include a
detailed explanation for each instance of unauthorized leave and any supporting documentation if
applicable and you may send the same through hard copy or email within said stipulated time.
Failure to provide a satisfactory explanation within the stipulated timeframe will compel the
company to initiate appropriate disciplinary action, which may include but is not limited to, a formal
suspension, or demotion in service or omitted salary increment or other punishment stipulated in
Model Code of Service Conduct. Any arbitrary reply in this matter will take further course of legal
measures for which you shall be render yourselves liable.

Subject: Outcome of Suspension Appeal and Disciplinary Action
Dear Mr. Bijit Mondal,
Pursuant to our last email dated we are writing to inform you of the outcome of the recent
suspension wages and subsequent appeal that you filed with the management. After careful
consideration of the circumstances surrounding your case, the management would like to
emphasize the importance of adhering to the company's policies and procedures,
particularly regarding leave management. Moving forward, any unauthorized leaves will be
subject to disciplinary action. As a measure of discipline, if there are any instances of
unauthorized leave, 4% deduction from your salary will be levied as aa measure of
disciplinary punishment.
We trust that this disciplinary measure will serve as a reminder of the importance of
following the established procedures and policies to ensure the smooth functioning of the
organization.
