1.

0 Purpose:

To provide appropriate guidelines for safe working in Confined Spaces present at the Site.

2.0 Scope:

Applies to all confined spaces and confined space entries at UFT Site .

3.0 Reference:

None

4.0 Definitions:

Acceptable entry conditions -

Conditions that must exist in a permit space to allow entry and to ensure that employees involved in a
permit required confined space entry can safely enter into and work within the confined space.

Authorized confined space resource -

An individual trained and knowledgeable in conducting hazard assessment and appropriate hazard
controls, and who is sanctioned by management to evaluate and authorize work in a confined space.
Confined space entry permits may be issued only by the authorized confined space resource.

Authorized entrant-

A trained employee who is authorized by the Confined Space Resource to enter a permit required
confined space.

Confined space entry attendant (Watcher):

The individual who is stationed outside the entrance to the confined space while the confined space is
occupied. The confined space entry attendant controls entry/exit into and out of the space. The
confined space entry attendant must maintain constant communication with the entrants.

Confined space entry work permit (permit) -

The written or printed document that is provided to allow and to control entry into a permit space.

Confined space -

A space that:

- Is large enough and so configured that an employee can bodily enter and perform assigned work.
- Has limited or restricted means for entry or exit, i.e., tanks, vessels, silos, storage bins, hoppers, vaults,
pipes, sewers, flues, tunnel, Shaft & pits.

- Is not designed for continuous employee occupancy.

- May have a hazardous atmosphere.

Hazardous atmosphere -

An atmosphere that may expose employees to the risk of death, incapacitation, impairment of ability to
self-rescue (that is, escape unaided from a permit space), injury or acute illness.

5.0 Policy Statements/Statement of Work:

5.1 The Project manager shall:

5.1.1 Ensure policy, procedures and management systems exist to comply with applicable government
regulation specific to confined space, and ensure employee protection from associated hazards are
incorporated into the organization's Occupational Health and Safety Management System.

5.1.2 Identify a competent resource to manage this process. This identified individual will assume
leadership and will work in conjunction with the staff to develop and ensure policies and procedures are
implemented to meet the provisions of the confined space standard for prevention of injury or death to
employees working in a confined space.

5.1.3 Audit for compliance.

5.2 Confined Space Entry:

5.2.1 All work performed in a confined space shall be considered as permit- required confined space.

5.2.2 Where possible, work conducted inside confined spaces shall be eliminated via engineering change
or modification.

5.3 Permit:

5.3.1 No confined space shall be entered until a confined space entry permit has been issued for the
work.

5.3.2 The confined space entry permit may be issued only by an authorized confined space resource.

5.3.3 The confined space entry permit shall be valid for a period of time not to exceed eight (8)
continuous hours. If work activity ceases and the entry site is unattended at any time, the permit is
cancelled and a new entry permit is required before work may resume.

5.3.4 The transfer of confined space entry permit between workers, work teams and shifts are
prohibited.

5.4 The confined space permit resource authorized to issue confined space work entry permits shall:

5.4.1.0 Ensure that each confined space is labelled and identified by site team/Project Team
5.4.1.1 A list of confined spaces at the Project Site to be maintained.

5.4.2 Conduct a hazard analysis assessment to determine the potential hazards which may be present,
develop control plans and establish entry work procedures or in a HIRA.

5.4.3 Ensure that employees identified by management to perform confined space work activity are
evaluated by medical staff and meet the physical requirements for the task and work environment.

5.4.4 Determine the method for communicating during a confined space work activity and identify
special tools, lighting, ventilation and personal protective equipment required. Also, identify an
employee emergency action plan, rescue process and equipment needed for such rescues.

5.4.5 Test for hazardous atmospheric conditions to determine acceptable entry and test continually
during confined space work activity.

5.4.6 Prohibit entry into confined spaces having, or reasonably suspected of having, dangerous
atmospheres.

5.4.7 Identify control measures to protect workers from hazardous internal/external conditions during
the period of work in the confined space, i.e., electrical lockouts, fluid and chemical detection,
entrapment, stored energy, air lines, disconnects, air/hydraulic power to cylinders, heat, steam, etc.

5.4.8 Prohibit work in an area where exit doors are locked unless the door is equipped with a panic
release device approved by national regulatory authorities and consistent with good safety practice, i.e.,
access-controlled areas such as computer rooms and other controlled -entry areas.

5.4.9 Establish, communicate and train employees in the confined space work permit process and in the
rescue and emergency action plans.

5.4.10 Educate, train and test employees for understanding prior to issuing a confined space entry
permit and authorizing employees to commence work in confined spaces.

5.4.11 Ensure that acceptable entry conditions exist prior to authorizing and issuing an entry permit.

5.4.12 Deny and/or approve all confined space entry permit required work. Indicate closure with a
signature on all cancelled or expired entry permits.

5.4.13 Authorize entrants’ entry to perform work in a confined space. While issuing the confined space
work permit appoint one rescue person if more than one person required to work in confined space

5.4.14 Isolate the work environment with a physical barrier and post signed, authorized confined space
entry work permit(s) at the site.

5.4.15 Ensure integrity of the confined space work permit system is maintained.

5.5 The confined space entry attendant (Watcher) shall:

5.5.1 Be stationed outside the confined space at all times while the confined space is occupied and shall
not enter in confined space in any condition.

5.5.2 Deny entrance to any employee other than confined space entrants listed by the entry supervisor
on the confined space permit.
5.5.3 Know at all times the number and identity of entrants inside the space.

5.5.4 Maintain constant communication with confined space entrants.

5.5.5 Demand immediate evacuation of the confined space any time conditions listed on the entry
permit change or there is evidence that hazards to the confined space entrants exist.

5.5.6 Upon conclusion of work in the confined space, or at such time as the confined space entry
permit is cancelled or expired, ensure that all entrants evacuate the space. The cancelled or expired
entry permit shall be taken to Security for closure by the authorized confined space entry resource.

5.5.7 The attendant shall be the rescue person during any emergency but ensure that he himself shall
not enter in the confined space and will pull out the person from confined space by Tripod if applicable.
If not possible then he shall call third person for help whether by emergency call or shouting.

5.6 The employee shall:

5.6.1 Perform work as outlined and as trained for the confined space entry work task.

5.6.2 Immediately discontinue work and exit confined space if there are any changes in the
environment.

5.6.3 Notify the confined space resource, his/her manager and security when cessation of a work
activity occurs.

5.6.4 Notify the confined space resource, security and the emergency response team when entry is
completed and the work permit status is closed.

5.7 Training:

5.7.1 Confined space entry training should be given to authorized entrants and confined space entry
attendants .

5.7.2 Authorized confined space resources are authorized to conduct confined space entry training.

5.7.3 Upon completion of Confined Space Entry course, employees must demonstrate competency to
conduct hazard assessment, identify controls, evaluate atmospheric conditions, determine acceptable
entry criteria and control the issuance of confined space entry work permits, before being included on
the list of authorized confined space resources.

5.7.4 Records of training completion shall be documented.

5.8 Acceptable Entry Conditions:

5.8.1 Entry conditions will be considered unacceptable and confined space entry will be denied if any of
the following conditions exist:

Oxygen levels are less than 19.5% or greater than 23.5% by volume,

Combustible gas is present at greater than 10% of its lower explosive limit (lower flammable limit),

If an airborne combustible dust obscures vision to five (5) feet or less,


If any atmospheric condition recognized as immediately dangerous to life or health (IDLH) is
present, or

If any other uncontrolled hazard exists within the space which could result in injury or death.

5.9 Rescue Person shall:

Keep the breathing apparatus for the rescue operation

Stand by full time till work permit closing

He will get continue communication with attendant for any rescue requirement.

If attendant is rescue person (if only one person enters in confined space) then he has to be ready to
take out the person during emergency.

If he is not capable to pull out the entrant person, he shall call at emergency.

This listing of hazard examples is not inclusive; specific work site hazards shall be identified by
conducting a hazards analysis. Hazard examples are:

- Flammable substances

- Oxygen-rich environment

- Toxic gas, fumes, or vapor

- Oxygen deficiency

- Presence of liquids internal to the work site and/or from remote locations

- Solid material which can flow

- Contamination

- Extreme heat

- Extreme cold

- Extreme noise

- Electricity
- Entrapment

- Stored and residual energy

- Compressed air

- Compressed gases

- Hydraulics

- Chemicals

- Moving equipment

- Hand tools
