Task 1: Emergency Response
Following are the points that how health and safety advisor managed effectively managed the
emergency.
1. “Safety advisor took the charge of the scene and asked bystanders to give him
some room” so that he could control the situation as mentioned in scenario.
2. “Safety advisor sought assistance the assistance of the Cinema manager and
team leader”, so that he could understand the emergency procedures of cinema.
3. “Safety Advisor asked for First-aid box” so that he could give first aid treatment to
the injured worker.
4. “Safety advisor examined the injured worker and noted his breath, abrasion”, so
that he could give treatment to injured worker accordingly.
5. “Safety advisor also called for emergency response”, so that injured worker could
get further treatment in time.
6. “Safety advisor made the injured worker comfortable as much as possible”, so
that the worker could get consciousness in earliest possible time
7. “Safety advisor asked a worker to clear and close the entrance hall”, so that he
could do proper investigation of accident.
8. “Safety advisor tried to telephone to Cinema manager, after multiple attempts he
left a voice mail”, so that he could inform the manager about situation.
9. “Safety advisor spoke to workers and allowed them to go home”, so that workers
could get some time to get themselves out of accident impact.
10. “Safety Advisor asked workers to write down anything they can remember about
accident”, so that he could get some help while investigating the accident.
Task 2: Culture
Following are the indicators of negative health and safety culture in water front cinema.
1. “No proper housekeeping was observed at waterfront cinema” as entrance doors
were dirty, represent a negative culture.
2. The entrance hall of the waterfront cinema had “overflowing waste bins as well as
tickets were littering on the floor” indicate a negative culture.
3. At the ticketing counter, a worker was speaking loudly and wasn’t paying attention to the
auditor as “there is lack of supervision” which represents a negative culture.
4. As mentioned in scenario, when auditor knocked at the door manager wasn’t even
interested in worker’s problems as “there is a work fears factor between manager
and workers”, indicates a negative culture.
5. When auditor entered in the manager’s “room he was watching a movie in his work
hours” which indicates a negative factor.
6. When auditor asked for a suitable space of work then manager said that “they don’t
have proper work place” which indicated a negative culture.
7. According to the scenario, when auditor gave a list of health and safety documents,
“manager wasn’t interested in doing work” which indicates a negative culture as
auditor listened words like burden.
8. In the scenario, when auditor entered into break room, “there were dirty dishes piled
on the work surface above the dishwasher”, indicates a negative culture.
9. As mentioned in the scenario, manager had forgotten that which file is about health and
safety of their organization so, “there is a lack of management commitment” and
indicates a negative culture.
10. In the scenario, when auditor asked the manager for the interview then he said that they
don’t have time for interview as “they are not cooperating with the auditor”. It is a
negative culture.
11. According to the scenario, auditor found that the policy was dated January, 2018 as
“there is a lack of suitable review of policy” which indicates a negative culture.
12. In the scenario, auditor also found that the only risk assessment was done in January
2020, as “there is no proper risk assessment” which is a negative culture.
13. According to the scenario, auditor also found that “the risk assessment page had a
different cinema name and address which is a mal practice” and is a negative
culture.
14. As per scenario, when auditor seek the assistance at accident place, “the manager and
team leader were absent during duty”, is a negative culture.
15. According to the scenario, none of the workers had first aid training, “there is a lack of
training” which is a negative culture.
16. When auditor asked for First Aid box, the first aid box only contained two plasters and a
bandage; “there is no compliance with the health and safety rules” which is a
negative culture.
17. When auditor tried to contact the manager multiple times, manager didn’t answer a
single call of the auditor, “there is a lack of leadership” which is a negative culture.
18. When auditor investigated the accident, workers were surprised about the investigation
of accident which shows that “accidents aren’t investigated in the cinema at all”
which is a negative culture.
19. As per scenario, workers told that manager hides the happening of accidents and thinks
that investigating an accident is waste of time, “there is no investigation culture”
which is a negative culture.
20. According to the scenario, young worker informed the team leader that he didn’t want to
use the stepladder as “an accident already happened with that stepladder”, which is
negative culture.
21. As mentioned in the scenario, “everyone knew that the stepladder was faulty” which
is a negative culture.
22. Young worker informed the team leader that he was afraid of height after that he was
forced to performed the task, “ignoring the workers complaints” is also a negative
culture.
23. When young worker asked for some to help him in performing task then “he was told to
work lonely” which also a negative culture.
24. As mentioned in the scenario, “the young worker was threatened to perform the
task” which is also a negative culture.
25. Witness told the auditor that the manager had a reputation of dismissing the workers
from the job which means “if u complain u loose the job” which is also a negative
culture.
26. As mentioned in the scenario that stepladder wobble as and at the same a child bumped
into the stepladder as “there were no any proper sings which show a mark working
in the area” which is negative culture.
27. As mentioned in the scenario, when auditor asked about the health and safety training,
the witness just said “none”, which shows that “there is no any health and safety
training” which is a negative culture.
Task 3: Health and safety management systems – awareness and communication
After thorough analysis of the scenario, it is clear that waterfront cinema has failed to effectively
communicate the following requirements and outputs of HSMS to its workers;
1. In the scenario, “The cinema has failed to give clear lines of communications
between top management and workers”, as manager is not willing to communicate
with the workers.
2. “The Cinema has failed to review its health and safety policy on time” as last date
mentioned in the contents of policy is January, 2018 as mentioned in scenario.
3. As mentioned in scenario, “the Cinema has failed to carry out risk assessment on
time”, as the only risk assessment was carried out in January, 2020”.
4. “The Cinema has failed to provide proper First-aid kit to tackle any mishap” as the
first-aid box contained only two plasters and a bandage in the scenario.
5. “The Cinema has failed to provide proper equipment at cinema”, as the wooden
stepladder was old and an accident had happened with that ladder as mentioned in
scenario.
6. “The Cinema has failed to provide supervision in the cinema”, as the team leader
was not supervising the activity and the manager was not present after accident as
mentioned in scenario.
7. As witness mentioned in the scenario that manager has a reputation for dismissing
workers for reporting accidents so, “There is a fear of reporting accidents and
Cinema has failed to tackle with this situation”.
8. “The Cinema has failed to provide cooperative top management”, as the manager
was uncooperative during health and safety audit.
9. “The cinema has failed to communicate health and safety policy”, as the policy was
not displayed anywhere in Cinema as well as policy was not accessible to workers in the
scenario.
10. According to thorough analysis of Scenario, there is no any culture to investigate any
accident despite the fact manager thinks it is waste of time, which shows that “Cinema
has failed to communicate the importance of investigation”.
11. As mentioned in scenario, “The cinema has failed to communicate the importance
of maintenance of equipment”, as they are using faulty stepladder.
12. “The cinema has lack of safety protocols in case of any emergency”, as workers
were unknown to the importance of emergency procedures in the scenario.
13. According to the scenario young worker was not aware of the risk associated with work
at height using wooden stepladder which shows that “there is a lack of
communication about hazards and risks between Cinema and workers”.
14. “The cinema has failed to communicate the importance of health and safety of the
workers”, so there is an obvious negative culture in the cinema regarding health and
safety in the scenario.
15. According to the scenario, “the cinema has failed to give health and safety
trainings”, as witness stated that they hadn’t received any kind of trainings.
Task 4: Top management commitment
A) Future Cinema Corporation (FCC) is at demonstrating top management commitment
due to following reasons;
1. “FCC’s corporate headquarters has a central health and safety department”,
which shows that they are serious regarding health and safety as mentioned in the
scenario.
2. According to the scenario, “FCC has a group health and safety director who is
responsible for developing cinema’s health and safety policy”, which also
shows that their top management is committed regarding health and safety.
3. “FCC has three RHSMs in the corporation who are responsible for
implementing the health and safety policy across the chain” in the scenario,
which shows that health and safety is one of the top priorities of top management of
FCC.
4. “FCC has 15 health and safety advisors who check arrangements regarding
health and safety”, which shows that they are serious about health and safety
according to scenario.
5. As mentioned in scenario, “FCC’s has certification against ISO 45001:2015”
which shows that they have strong management commitment regarding health and
safety.
6. “FCC also help other organizations to gain certification against the same
standards”, which shows a commitment to promote health and safety among
industry.
7. FCC’s top management is committed regarding health and safety, so that “their
staff is competent and experienced”.
8. FCC’s top management is committed regarding health and safety as “their safety
advisors travel across the country to check the arrangements regarding health
and safety.” As mentioned in scenario.
9. As mentioned in scenario, “FCC’s top management has hired senior
management so that there are clear line of communications”, which shows
FCC’s the top management commitment regarding health and safety.
10. According to scenario, “FCC’s top management ensures that risk assessment
and health and safety audits are being carried out regularly”, which shows the
top management’s commitment regarding health and safety.
11. “FCC is providing resource to ensure health and safety across the chain”,
which indicates the dedication of top management regarding health and safety.
12. FCC has structured approach to incident response and investigation which shows
that top management is committed to continuous improvement regarding health and
safety.
B) If labour inspector were to visit the workplace(Water Front Cinema) in case of serious
injury to worker, then he could impose following actions on Water Front Cinema;
1. He could issue prohibition notice to the Cinema.
2. He could issue an improvement notice to the Cinema.
3. He could cancel the license of cinema.
4. He could impose legal fines, compensation
5. He could impose imprisonment to the manager or team leader of the cinema.
Task 5: Administrative Control measures
Following are the control measures that could be put in place at water front cinema which could
prevent the accident;
1. “Water Front Cinema’s health and safety policy must be up to date and must be
accessible to employees”, so that employees can have knowledge about Cinema’s
Health and Safety policy.
2. “Water Front Cinema risk assessment must be done regularly”, so that risk of
accident can be reduced.
3. “Water Front Cinema must give induction trainings to the employees regarding
health and safety”, so that they can be aware of the hazards related to different
activities.
4. “Water Front Cinema must implement proper supervision during the activity”,
so that they can perform their task properly and in case of accident they could take
necessary precautions.
5. “Water Front Cinema must prepare their emergency procedures”, which can be
implemented in case of any accident.
6. “Water Front Cinema must do proper inspection regularly”, so that they can
have proper knowledge regarding risks and hazards in the organization.
7. “Water Front Cinema must implement safe system of work in the organization”,
so that risk of accident can be reduced.
8. “Water Front Cinema must have clear line of communication between top
management and workers”, so that they can give feedback for the improvement of
health and safety within the organization.
9. “The disciplinary policy must be reviewed”, so that employees are not in fear to
report incidents.

understands their role in maintaining a safe working environment.
Task 6: Individual and job Factors
A) Following individual human factors are likely to have influenced the behavior of
injured worker;
1. “Young worker was inexperienced as he was not capable of doing the
activity properly”, so that this factor may have influenced the behavior of injured
worker.
2. “Young worker had not proper trainings to perform activity”, so that injured
worker’s behaviour might be in influence.
3. “The team leader's behavior, including laughing at the worker's concerns
and telling them to stop complaining”, might have influenced the behaviour of
injured worker.
4. “The injured worker may have lack of confidence to refuse the task or
suggest an alternative”, this factor may have influenced the behaviour of injured
worker.
5. “The injured worker was afraid of height”, as he informed the team leader.
6. “The injured worker’s stress level may have been increased due to pressure
to complete the task”, this factor might have influenced the injured worker’s
behaviour.
7. “The injured worker had fear of authority”, as his manager had reputation to
fire the workers who report.
B) The following job factors are likely to have influenced the behavior of the injured
worker;
1. “The injured worker had not received any health and safety training”, as he
was unaware risk associated with using stepladder.
2. “No proper supervision was provided to the young worker”, as his team
leader was not there to supervise the task.
3. “The task of hanging the banner was poorly designed”, as no proper
equipment was provided as well as no process was carried out.
4. “The team leader put pressure on the young worker to complete the task
quickly”, this might have influenced the injured worker.
5. “The worker may have been unclear about their responsibilities”, this might
have contributed to behaviour of injured worker.
6. “The Cinema’s negative culture regarding health and safety”, might have
influenced the injured worker.
7. “There was no proper safe system of work to carry out the activity”, as the
area of activity was not isolated, this factor might have influenced the injured
worker.
8. “There was no proper suitable alternative to the wooden stepladder”, this
might have influenced the young worker.
9. “No one was there to assist the young worker during the activity”, this
factor might have influenced the young worker.
Task 7: Risk Calculation
We can calculate the level of risk by using risk matrix formula (R=L*S).
A) Risk level was not acceptable when the ladder was used for hanging the banner.
According to the given matrix, “the chances of falling from the ladder were
high” as the ladder was old and wooden. So that likelihood is 2.
“The chances of having injury to the worker were major injury” as he was
working at height without proper safety so that severity is 3.
When we apply the formula (R=L*S), then we get
R=2*3
R= high (red).
As per calculations, “risk level is high which falls in red portion of the
matrix” which indicates that risk must be reduced.
B) Risk level was acceptable, if a different form of equipment (such as mobile scaffolding)
was to be used.
If the young worker used other safe equipment then “chances of falling would
be very unlikely”, likelihood would be 1, according to the matrix.
“The chances of having injury to the worker would remain major injury” as
he was working at height so that severity would remain the same which is 3.
When we apply the formula (R=L*S), then we get
R=1*3
R= medium (yellow)
As per calculations, “risk level is medium” which is acceptable.
By using safe equipment the risk level of banner hanging activity could be
reduced which is acceptable.
