Please note that bill deposit shall be retained equivalent to your average one month bill and will be
refunded according to the following conditions:
* Upon termination of your service contract;
* Prior to the termination of your electric service; provided that you have paid your electric bills on or
before
its due date for 36 consecutive billing months (3 years);
* And, One (1) year after the payment of your bill deposit, and every year thereafter, if your existing bill
deposit is less than your actual average monthly bill for the preceding 12-month period, the excess shall
be
refunded to you in the form of a credit adjustment to your bill.
Your payment for the foregoing amount can only be accepted after you have done all the necessary
works
required by our Distribution Engineer and you have submitted the following:
a. Original copy of Electrical Permit
b. Copy of Land title, Tax Declaration or Deed of Sale (bldg. and lot) or Certificate of Occupancy
IF UNDER CORPORATION'S NAME: SEC Registration and Company Tin Number, Secretary’s
Certificate or
Board Resolution and Authorized Representative's Valid Ids
IF UNDER PERSON'S NAME: Copy of 2 Valid Ids, Birth Certificate (if single) or Marriage Contract if
married),
Notarized Special Power of Attorney and representative's Valid Ids (other than applicant)
IF SINGLE PROPRIETORSHIP: DTI BUSINESS PERMIT or MAYOR'S PERMIT, Copy of 2 Valid Ids,
Notarized
Special Power of Attorney and representative's Valid Ids (other than applicant)
ADDITIONAL REQUIREMENTS IF RENTED : Contract of Lease, Notarized undertaking and
lessor's Valid Ids
