Nursing Care for Vision Impairment

The client reported pain and blurry vision with a haze over their eyes. On examination, the client was diagnosed with disturbed visual sensory perception related to altered sensory reception. The goal was for the client to participate in a therapeutic regimen to maintain their current vision field and acuity with minimal loss after 8 hours of nursing intervention. Interventions included encouraging the client to express feelings about potential vision loss, educating on measures to assist with any visual limitations, and assisting with administering eye drop medications to lower intraocular pressure and prevent further vision loss.

RATIONALE

EVALUATION After 8 hours of nursing intervention the patient were

Participate in therapeutic regimen. Maintain current visual field/acuity with minimal loss

Cloudy pupils Limited vision and Headache

Although early intervention can prevent blindness, patient faces the possibility or may have already experienced partial or complete loss of vision. Although vision loss cannot be restored (even with treatment), further loss can be prevented Reduces safety hazards related to changes in visual fields/loss of vision and papillary accommodation to environmental light

Participated in therapeutic regimen. Maintained current visual field/acuity with minimal loss

Recommend measures to assist patient to manage visual limitations, e.g., reducing clutter, arranging furniture out of travel path; turning head to view subjects; correcting for dim light and problems of night vision

Collaborative Assist with administration of medications as indicated ordered Pilocarpine hydrochloride

Timolol maleate
