to law, depose and say that:
1. I am the father of DAVID M. NAMBATAC, who is applying for
college scholarship with the CHED Student Financial Programs;
2. I am a Tricycle Driver by profession, I earned a monthly income of
P15,000.00 and my wife is a homemaker;
3. I am exempted to file my Income Tax Return for the year 2017-2018
with the Bureau of Internal Revenue;
4. I am therefore executing this affidavit to attest to the truth of the
foregoing facts and for whatever legal purpose this may serve;
