SWORN DECLARATION
I, ______________________, Filipino, of legal age, single and a resident of
Balangasan District, Pagadian City, Philippines, after having been duly sworn to
in accordance with law, hereby depose and state, that:
1. I am the authorized representative of __________________________,
registered with the Bureau of Internal Revenue under Tax Identification
Nos. ______________________________ with principal business office address
at Salera St., Purok Spring Sta. Lucia District, Pagadian City;
2. The Corporation has been using Manual Book of Accounts for its SALES
and PURCHASE transactions;
3. The Corporation has applied a Permit to Use Loose Leaf on SALES and
PURCHASE Book of Accounts pursuant to Revenue Memorandum
Circular No. 68-2017 dated 10 August 2017. In compliance therewith,
hereto submitted are the requirements together with the foregoing Sworn
Declaration, in lieu of the investigation to be conducted by virtue of RMC-
13-82;
4. The Corporation is committed to permanently bind the loose-leaf forms
within fifteen days after the end of each taxable year or upon the
termination of its use;
5. I am executing this affidavit to prove the veracity of the facts mentioned
herein and for whatever legal intents and purposes this affidavit may
serve best.
