AFFIDAVIT OF DISCREPANCY
I, SANCHO F. ORAA JR., of legal age, widower, Filipino and residing at
Brgy. Sta. Rosa, Mulanay, Quezon, after having been duly sworn to in
accordance with the law, do hereby depose and state under oath:
1. That I am the legitimate husband of late MERLY ARENAL
ORAA/MERLE LEONA ARENAL and we got married on June 2, 1990
in Mulanay, Quezon. Photocopy of our Marriage Contract is hereto
attached as Exhibit “A”;
2. That during our marriage, we have been blessed with five (5)
children namely Mark Joseph Arenal Oraa, Mary Joy Arenal Oraa,
Glory Mae Arenal Oraa, Mark Anthony Arenal Oraa and Mark Angelo
Arenal Oraa. Photocopy of their Birth Certificate are hereto attached
as Exhibit “B” thru Exhibit “F”;
3. That our children Mark Anthony Arenal Oraa and Mark Angelo
Arenal Oraa are twins who were born February 20, 2002;
4. That during their birth, it was Mark Anthony Arenal Oraa who came
out first from their mother’s womb followed by Mark Angelo Arenal
Oraa;
5. That the birth order of Mark Anthony Arenal Oraa and Mark Angelo
Arenal Oraa in their Certificate of Live Birth were erroneously
written as no. 05 and no. 06 instead of their true and correct order of
birth as No. 04 and No. 05;
6. That I am executing this Affidavit of Discrepancy to attest to the
truthfulness of the foregoing facts and to explain the discrepancy in
the Certificate of Live Birth of my children Mark Anthony Arenal Oraa
and Mark Angelo Arenal Oraa as regards to their birth order;
7. That I am also executing this Affidavit of Discrepancy for the sole
purpose of claiming death benefits.

Oraa and our Marriage Contract and the MERLY ARENAL ORAA that
appears on the Certificate of Live Birth of our 2 children namely
Mark Joseph Arenal Oraa and Mary Joy Arenal Oraa and all other
other pertinent documents attached herewith refer to one and the
same person and to no one else but my wife.
