AFFIDAVIT OF CONSENT
I, GENALYN CARREON of legal age, Filipino/British and residing at B12 L7
Park Spring Subd. San Antonio, San Pedro Laguna/17 Mowbay Rd. Bedford, United
Kingdom, after having duly sworn to according to law hereby depose and state:
1. That I am the daughter of ROFOLFO N. CORTEZ and AMPARO D.
CORTEZ who are both senior citizens and who are both ill;
2. That I am appointing RANDY OPERARIO, likewise of legal age, as their
temporary companion and that he voluntarily presented himself to be as such;
3. That the purpose of the appointment is to keep my parents safe because I would
be travelling back to the United Kingdom to continue my work there;
4. That verified this affidavit, to be a matter of record, now and for all times, that I
am giving my consent to RANDY OPERARIO to take care of my parents
RODOLFO N. CORTEZ and AMPARO D. CORTEZ while I am away.
5. That, I am executing this affidavit to attest the truth of the above mentioned for all
legal intents and purposes this may serve.
