By submitting this Nomination Form, you understand and agree the following terms and conditions:
a) Please complete this form in full by entering correct details in each box. It is the sender’s responsibility to inform nominees of all details.
b) All candidates above 50 yrs of age, should produce a medical fitness certificate which is valid for not less than 6 months on the nomination date, from a PDO approved clinic.
c) PAYMENT: Proof of payment by cash customers must be included with nominations. (Confirmation shall be given only on receipt of payment.)
d) Cancellation must be faxed or emailed to Sheida International 72 hours before the start of the course to avoid payment of course fees.
e) Nominees attending DDC training should be medically fit as specified in SP 1230. A vision test will be conducted during training. Failing the vision test is considered a No Show. “The eye
test is an important pre-requisite for attending defensive driving training. If you fail the eye test on the day of the course you will be charged for the full course but cannot continue. You are
therefore encouraged to have an eye test at least 72 hours before your course either at TATI or SHEIDA as the results remain valid for three Months, the eye test can be taken between
the hours of 9am and 11am with no appointment, just call into their training premises
f) Supervisors should refer the nominee to an approved doctor / optometrist if there is any doubt about the nominees’ Physical or medical fitness or vision
g) Muscat course starts at 7am-- Interior course starts at 8am and 1pm reporting fifteen minutes after course start time will be considered as” No Show” and the full course fee is payable)
