LIVESTOCK INSURANCE SC HEME-JAMMU&KASHMIR
Introduction: Livestock lnsurance scheme has been introduced by the State Government as
a part of CSS (NLIV) wherein a Household (Husband, wife and dependent
children) can insure up to five animals (cows / buffaloes, etc). lndividuals above
18 years of age with valid Aadhar number are considered as separate family for
the purpose of Livestock lnsurance.
Duration: One to three years.
Coverage: Death of the insured animal (due to disease / sickness or accidental) is covered
by the scheme immediately after the start of the insurance. Animal theft is not
covered under this scheme. Fifteen (15) days cooling period is required for
claims arising out of natural death.
Documents required for Livestock lnsurance:
1. Clear photograph of the animal in standing position from all sides along with
the beneficiary and photo of the animal tag (12 digits bar code).
2. Animal health certificate (issued by the Veterinarian)
3. Aadhar Number of the Beneficiary 4. Premium Amount
5. SC /ST/ BPL certificate (to whom applicable)
Whom to contact: Beneficiaries need to contact their nearest Veterinary Hospital I Bank
Branch.
Claim process and mentation required: The body of the deceased animal has to be
kept safe for 24 hours for inspection by the investigator. The death should be
immediately reported to the nearest Veterinary Surgeon and the Claim center
(branch / number). Following documents are required for claim processing:
1. Claim Form 2. lntimation letter to lnsurance Co. 3. Ear Tag
4. Post-mortem report (Veterinarian's) 5. Copy of the Health certificate
6. Discharge Voucher 7. Copy of Aadhar Card 8. SC/ST/BPL certificate
9. Bank Passbook or Cancelled Cheque (Jan Dhan Account shall not be
accepted)
