Document Requirements to Obtain a Driverʼs License/State ID Card
The Acceptable Identification Documents chart (reverse) lists documents that may be used as forms of identification to provide
proof of legal name, date of birth, Lawful Status, Social Security number and residency when applying for a driver’s license (DL) or
identification card (ID).

Please note the following:


• Signature comparison is required in the verification process.
• The number of documents required depends on whether you are applying for a DL/ID for the first time, or requesting a
duplicate or corrected DL/ID.
• One document may satisfy more than one group.
• If you are a new resident, you must surrender all out-of-state licenses/ID cards before issuance of an Illinois DL/ID.
• If you are applying for a VISA/NONVISA DL/ID, you do not have to present documents from Group C. Instead, you must
sign a declaration at the time of application that you are ineligible to obtain a Social Security number.
• If your Illinois DL/ID has been expired for over a year, you may only need a document from Group A and D.
First-time Illinois DL/ID or First-time REAL ID (even if you hold or have held a standard Illinois DL/ID)
If you are applying for a driver’s license or ID card for the first time in Illinois, you must present:
