First Quarter Performance Task: “I” in Faith: Mental Wellness Plan


Target Date of Submission: September 20, 2024

Performance Task Scenario: In order for us to maintain our stamina in addressing these problems,
it is highly necessary to keep our well-being in check. Hence, you are going to come up with a
Personalized Mental Wellness Plan which aims to guide you in nourishing your mental, emotional,
cognitive and physical health amidst the crisis we face.

Culminating Performance Standard: The learner will be able to construct a creative visualization
of his/her personal development by undergoing a series of personal assessments that include
identifying personal stressors, social influences, and decision-making points as a middle and late
adolescent.

Mechanics:
1. The “I in Faith” Mental Wellness Plan is an articulated output with CLF 3 and MIL. This is a
component in the “Faith in Action” Infographics in CLF 3. Personal Development will assess
how you, as a middle or late adolescent, can create healthy coping mechanisms amidst
the challenges faced individually, and collectively.
2. To accomplish this task, you shall reflect and answer the given mental health plan
template. (See Annex A)
3. In creating the Mental Wellness Plan, you have the option to choose an editing software,
such as Canva, Photoshop, etc., based on your familiarity and preference.
4. The final output shall be in image format (.jpg, .jpeg, or .png). Together with the Faith in
Action Infographics, submit your accomplished mental wellness plan in Quipper Essay,
entitled, PerDev_1st Quarter PT_Mental Wellness Plan.

1. Based on the Faith in Action Infographics, individually, you are going to craft your
personalized Wellness Plan using healthy ways of coping. In this way, you have a guide
on how to protect and take care of yourself amidst the social issues collectively faced.

Your Mental Wellness Plan should be S.M.A.R.T (Specific, Measurable, Attainable,

Specific- Your plan should point out specific tasks you shall do to take care of
