Subject – Legal Notice
Dear Sir,
Under the instruction and on behalf of my client Dhananjay Arvind Chaudhari, I Advocate Shubhra Jyoti Saha, do
hereby serve upon you, Reliance General Insurance Company Limited, the following legal notice:
1. That my client on August 30, 2023 purchased a health insurance policy from Policy Bazar, issued by your
concern, with the policy No. 920222328680535447.
2. That the insurance covering medical expenses incurred by my client and his dependents, including his son
Bhavesh Dhananjay Chaudhari.
3. That the son of my client was admitted to Siddhivinayak Hospital, Chopada Jin, Shirpur from 29 th October
2023 to 04th November 2023 due to Dengue Fever.
4. That the Hospital provided a total bill of Rs. 39,390/- (Rupees Thirty Nine Thousand Three Hundred Ninety
Only) bearing Receipt No. 659 and my client diligently settled all medical bills incurred during his treatment.
5. That my client duly submitted all the bills and details and sent the forms of the claims to your concern.
6. That despite my client’s cooperation throughout the claims process, including responding promptly to queries
and facilitating investigations, my client was shocked to receive a rejection of his claim on grounds of alleged
misrepresentation and discrepancies.
7. That the claim rejection, as detailed in your communication, cited Clause 5.1.1 of the policy terms and
conditions regarding disclosure of information.
8. That however, my client vehemently refutes any accusations of misrepresentation or non-disclosure and asserts
that all necessary information was provided in good faith.
9. That furthermore, my client’s attempts to seek clarification and resolution from both Policy Bazaar and your
concern have been met with unsatisfactory responses, exacerbating my distress and financial burden.

10. That even after initiating proceedings with the Ombudsman, the subsequent reopening and subsequent
rejection of the claim by Policy Bazaar has only added to my client’s frustration and disillusionment.
11. That this ongoing situation has caused my client significant mental distress and financial burden.
12. That by your act and conduct my client had got agonized which is not at all tenable under the law and a proof
of unfair trade practice.
13. That being deprived of the facilitated service my client intimated your concern on several occasions in respect
of the above-mentioned issue, however you have acted like flogging of a dead horse in order to provide any
resolution.
Thus in order to resolve the difficulties of my client, I call upon you to duly look into this matter with foremost
importance to provide the proper and facilitated time towards my client to provide justification and reimburse
the full billing amount of Rs. 39,390/- (Rupees Thirty Nine Thousand Three Hundred Ninety Only) within the
insurance claim, along with Rs. 2,359/- (Rupees Two Thousand Three Hundred and Fifty Nine Only) for the
legal fees incurred by my client and a compensation of Rs. 30,000/- (Rupees Thirty Thousand Only) for
service deficiency and harassment suffered by my client for not providing proper services to my client,
otherwise my client will have no other alternatives to file a case in appropriate forum for legal remedies.
I would direct you to provide all the expenses in lieu of the possible recourse towards my client.
Kindly be advised that a subsequent or reminder legal notice may be issued if you fail to respond to this legal
notice or if any further circumstances necessitate.
I look forward to your reply and a resolution to my client’s problem and will wait until 7 (Seven) days of the
receipt of this notice, failing which my client shall be constrained to initiate against you for redressal of the
aforesaid grievances and recovery of the aforesaid amount such proceedings, both civil and criminal as are
warranted by Law exclusively at your own risk, cost, responsibility, and consequences which please note.
