1. Allotment Letter from KNRUHS, Warangal.
2. KNRUHS Online Provisional Verification Report.
3. Original Admit Card (NEET). & Original Rank Card (NEET).
4. Original Memorandum of Marks S.S.C / CBSE (10th Standard) showing the Date of Birth.
5. Memorandum of Marks. Intermediate (10+2)
6. Equivalence Certificate (Intermediate) in case of state other than Telangana
7. Transfer certificate (TC).
8. Original Bonafide Certificates as follows:
a. VI to X Classes b. Inter I & II Year
9. Caste certificate in original from M.R.O (if required).
10. Xerox Copy Photo Identity Proof (Aadhar Card & Pan Card) (Student’s & Parent).
11. Latest Ten (10) Passport size photographs.
12. Discontinuation Bond MBBS (on non-judicial stamp paper of Rs.100/) (Duly Notarized) as per for KNRUHS
13. Undertaking Bond for B.G. submission (B & C Category )
14. Proforma for certificates genuine Bond (Non-Judicial Stamp Paper for Rs.100/-)
15. Bank Guarantee (BG) (From any Nationalized Bank) for B & C Category only.
16. NRI Declaration Form along with required documents
17. Three (3) Sets of Xerox Copies of all the above Certificates in the above order along with pdf soft copy (1 set self attested)
18. Fee Demand Draft in favour of “CHALMEDA ANAND RAO INSTITUTE OF MEDICAL
SICENCES” Payable at Karimnagar
