User Feedback
User feedback is defined as “qualitative and quantitative data from customers on their
likes, dislikes, impressions, and requests about a product” (Pendo, 2021). It is considered
an essential tool in improving a website; it allows the site visitors to commend the
website's strengths while also identifying its areas for improvement.
The Internet gives plenty of options in gathering the audience's feedback. One is through
Google Forms, which is widely popular due to its functionality and user-friendliness. We
can use it for academic purposes, surveys, online registrations, feedback, and customer
support.
Creating an Online Survey with Google Forms
1. Sign-in to your Google account.
2. Go to https://docs.google.com/forms/ and start a blank form.
3. Click Untitled Form to edit the title of your form.
4. Click Untitled Question to enter a question.
5. Use the dropdown box to change the question type.
a. Short answer. It allows a one-line response.
b. Paragraph. It allows a long form of response.
c. Multiple choice. It provides a list of options in which one can only be
selected.
d. Checkboxes. It provides a list of options in which more than one can be
selected.
e. Dropdown. Similar to multiple choice, only one can be selected among a list
of options. It is best used when there are a lot of possible options available.
f. File upload. It requires a file to be uploaded.
g. Linear scale. It sets a numerical scale with labels for the lowest and highest
options.
h. Multiple choice grid. It provides similar options to a list of sub-questions.
i. Checkbox grid. It functions similarly with the multiple choice grid, but more
than one option can be selected.
j. Date. It asks for a specific date in the MM/DD/YYYY format.
k. Time. It asks for a specific time.
6. Add response options if applicable.
7. Optional: If it is a question that must not be skipped, click Required.
8. To add a new question, click Add question in the Insert toolbar. Note: The
Insert toolbar also allows you to add a title and description, image, video, or
section.
9. Repeat nos. 4-8 until you complete the form.
10. Optional: Click Customize Theme to insert a header, change the theme and
background color, or pick another font style.
11. Optional: Click Settings to check the form settings.
12. Click Preview to take a final look at your form.
13. Click Send to share the form.

VI. Performance Task
Online Campaign Evaluation
1. Using Google Forms, create an online survey to evaluate your online campaign
(ICT Project for Social Change).
2. Upload the group’s infographic in the form.
3. The online survey must include the following items:
a. The opening statement engages the audience.
b. The issue is well-stated.
c. There is enough data to support the existence of the issue.
d. Various examples presented elaborate the issue.
e. The issue aligns with the audience’s values, concerns, or self-interest.
f. The call to action is clear and direct.
4. Each item will be scored using the scale: strongly agree, agree, disagree, and
strongly disagree.
5. Send the form to 50 respondents and provide a summary report of the results.
