Thank you for taking the time to participate in the Risk Assessment Survey. The

goal of this assessment is to identify threats and hazards to better improve our

response to risks that can impact the Royal Clinics.

*Your answers will be kept anonymous*

We kindly ask you to read the following definitions before completing the survey:

• Risk: the effect of uncertainty on the Royal Clinics’ ability to meet its

objectives

• Likelihood: the probability that something might happen

• Impact: the most probable results of the potential incident

Your detailed responses will help us identify and implement effective risk
