INTRODUCTION
A record is a permanent written communication that
documents information relevant to a client's
healthcare management .
Recording is an important & necessary aspect of
health care.
Clinical teaching is the individualized or group
teaching of nursing students in clinical settings by
nurse educator, staff or clinical nurse manager to
enhance the students' knowledge related to patient's
problems and care.
Process recording is a method of clinical teaching.
