Sub: Letter to informe about the Lost ATM Card and issue me
new one

Respected Bank Manager,

This is to inform you that I recently lost or misplaced my ATM


Card which is connected with my savings account with A/c No.
7659000100071750 last month it happened that I lost my ATM
card somewhere in traveling.

There is a huge amount of money in my account and prompt


action is required to stop all the transactions made from my
account using that ATM card. You are requested to block and
immediately terminate all the transactions which are to be made
from this card. I request you to please cancel that card and issue
me a replacement card as early as possible.

I would really appreciate your prompt attention to my request. I


shall be thankful to you for this kind favor. Thanking you in
advance.
