Needs of Dose Calculation
Infant, adult and elderly dose calculation depending on the
weight
Without knowing how to administer and monitor medicines and
intravenous therapy it is not possible to perform this task.
Age and sex also plays a role
Sometimes a doctor is not present at the site when the patient
is in need of a medicine.
Knowing the skill of drug calculation makes a nurse confident

2. Calculate the IV flow rate & drip rate for 250 mL of 0.9% NaCl IV over
3 HOURS .Infusion set has drop factor of 60

Tips for Correct Calculations:
If necessary, convert between units.
All measurements are in the same units.
In most cases, it is best to convert a larger unit to smaller units
Always check for reasonableness of your answer.
When solving dosage problems for drugs supplied in tablets or
capsules, the quantity, Q, is always 1 because the supply dosage
is per tablet or per capsule

1. Consultant writes an order for inj pipzo 300 mg iv to patient of age 2 yrs
Pharmacy dispenses 1 vial of 1.125 gm. Dilute with 5 ml.
2. Advised dose for inj Augmentin is 125 mg. you are having 600 MG in 1 vial.
3. MD writes an order of calcium gluconate 2 gm .Dose in hand is 1375 mg in 10
ml.
4. Pediatric Patient with Allergic fever is ordered Inj Paracetemol 50 mg .Dose in
hand is of 2 ml which contains 300 mg.
5. The Oncologist ordered Inj Oxaliplatin 55 mg. Each 12ml contains 100 mg.
6. Nephrologist ordered tab Arkamin 0.15mg. dose in hand is 100 mcg.

7. A solution of Ferric carboxy maltose inj. Was supposed to administer to
pateint, but the solution was very much viscous, so the Consultant ordered
to decrese the concentration to 1 mg/ ml. How will you manage?
8. Inj. Amikacin is order to pediatric patient with wt 2.5 kg . Ordered dose
is 15mg/kg. How much ml will you administer to patient if Amikacin 500
mg is available in 2 ml.
9. Inj. Meropenem is order to pediatric patient with wt 3.5 kg . Ordered
dose is 25mg/kg. How much ml will you administer to patient if
Meropenem 500 mg in powder form.
Syringe Pump
1.Consultant Ordered Inj. Streptokinase 2.5 Lakh unit stat dose.
Followed by 1 lakh units / hour, via syringe pump.How many ml will
you administer. ( Dose Available- 15,00,000 IU )
2. Consultant Ordered Inj. Polymixin B 1 Lakh unit stat dose.
Followed by 50, 000 units / hour, via syringe pump. How many ml
will you administer. ( Dose Available- 5,00,000 IU )
If consultant ordred inj N.Acetyl cystine to 65 kg patient as Inj acetyl cystiene
125 mg /kg f/b acetyl cystiene 100 mg /kg f/b acetyl cystiene 50 mg
/kg How many ampoule you will administer to patient.

patient whose wt is 10.6 pounds .
3. Calculate dose for Ceftriaxone ( adult dose is 1000 mg) for a
patient whose wt is 6.8 kg .
Fried's rule
