AFFIDAVIT
I, WILFRANDO ARELLANO, of legal age, Filipino and resident of
Mindanao St., San Juan, Malolos City, Bulacan, after having been duly sworn
to in accordance with law, do hereby depose and say:
1. That on December 26, 2013, I was issued by Villarica Pawnshop
Pawn Ticket No. 092088 for my loan of P3,500.00 and will mature on January
26, 2014;
2. That however, I am now physically disable due to the stroke illness I
had suffered recently and I cannot affix my signature on any papers as its
consequence;
3. That my signature can only be identified thru my thumb mark;
4. That from now on, I have to use my thumb mark as my identifiable
signature in any and all documents;
5. That I am executing this affidavit to attest to the veracity of the
facts contained herein and for whatever legal intents and purposes it may
serve.
IN WITNESS WHEREOF, I have hereunto set my hand this 16 th day of
