1. Pl. pay registration charges amounting to Rs. 2000.00 by cash or DD favouring
UGVCL payable at Mehsana
2. Copy of last regular monthly energy bill.
3. Attested true copy of the fresh certificate of incorporation issued by the Registrar
of the company.
4. Certified copy of Memorandum of Articles of incoming party.
5. Attested true copy of Registered partnership deed.
6. Documentary evidence to show the dissolution of the old partnership and
settlement with the outgoing partners.
7. Certified copy of Registered Sale / Purchase deed.
8. Undertaking duly notarized on a stamp paper of Rs.100/- as per the attached draft
to bear the liabilities arise out of the agreement executed by the outgoing
consumer with the Board from incoming party.
9. Undertaking duly notarized on a stamp paper of Rs.100/- as per the attached draft
by the outgoing party to the effect that they have no objection in transferring
security deposit paid by them to the new party.
10. Certified true copy of Board’s resolution specific authorizing the Director/Person to
sign the undertaking with specimen signature.
11. Please affix common seal (embossed seal) on the undertaking in case of Private
Limited/Limited concern.
12. Undertaking to abide the court decision in case of any pending legal cases.
13. The copy of original Agreement signed with the Board.
14. NOC from outgoing party for change of name.
15. Order of Hon’ble High Court.
