The IRS requires that the University of Kentucky provide a 1098-T statement to you for your use in completing IRS Form 8863.
The amounts and calculations used to determine the credit are the decision of the taxpayer after the consideration of relevant
IRS regulations, Form 8863. If you have questions pertaining to the calculations of your 1098-T, you may email the University
of Kentucky Student Account Services tax office at taxcredit@lsv.uky.edu.
If you have questions pertaining to filing your taxes using information on your 1098-T you should consult a tax consultant.
For official IRS documents obtain a copy of IRS Publication 970, available on the web at http://www.irs.gov. To order forms or
instructions call (800) 829-3676. To ask tax questions, call the IRS at (800) 829-1040.
NOTE: Student's SSN field is blank if the social security number has not been recorded on the student's permanent
record. However, the information on the 1098-T form has been reported to the Internal Revenue Service.
