To be used by students financially dependent on the support of others.
Please submit this form alongside your documentation of funding. Your documentation
must include:
The sponsor's name
Be dated within the last 12 months
Be written in English
If you plan to have multiple sponsors, please include a separate affidavit for each sponsor. Each personal sponsor must provide
a bank statement or letter from the bank showing U.S.D. or the equivalent amount in foreign currency that is needed. The
sponsor's name on both this affidavit, and the official bank statement must be the same.
