Sub: Prayer for interdistrict transfer of an Asst. Teacher in special medical


emergency for save her husband and father-in-law from serious health risks.

Esteemed Sir,

With due respect I Smt. Bharati Sahu, Asst Teacher of Panchasakha Govt.
High School Turusinga-Srimula, Kamakhyanagar, Dhenkanal, wish to have my
transfer from Panchasakha Govt. High School to any of the following four High
Schools such as ;- 1) Athanga Govt. Girl’s High School, Athanga 2) Ayatpur Govt.
High School, Ayatpur 3) Gopal Smruti Vidya Pitha , Siddheswarpur 4) Ratnakar
Bidya Mandira, Biribati or any highschool nearer to Kandarpur of district Cuttack
due to the following reasons :-

1) My husband is an OPTCL employee and posted at Tirtol of Jagatsinghpur


district and most unfortunately he is suffering from fits and going through a
prolonged treatment. He is now under leave also for this reason. Copy of his
medical reports are enclosed herewith for your kind reference. He needs
personal care and attention from me as there is no one in the family to look
after him.
2) My father-in-law is more than seventy years of old senior citizen and has
been facing several geriatric problems. He has already faced one stroke
earlier and was admitted in medical for it. He also needs proper medical care
and most unfortunately my mother-in-law is not alive. I have also submitted
his medical records.
3) I wish to inform you that my father-in-law is an ex-military person who has
served Indian Army from 1971 to 1995. He faced a serious accident and was
under treatment from SVNIRTAR (Swami Vivekananda National Institute
of Rehabilitation Training and Research). But it is very pathetic that he is
now very much helpless and expecting practical service from his son and
daughter-in-law.
4) It is pity to inform you that my mother-in-law was a CKD patient and was
went through dialysis. But I could not serve her and she was died on dt.
30.12.2019. Her death certificate is also attached with this application.
5) I wish to inform you that I am staying away from my husband and Father-in-
law with my two children aged about 4 year and 1 year respectively but
there is no one to look after them in such a difficult situation in my absence.
So it is very much essential for me to be transferred from here to any of the
above four high schools nearer to Kandarpur Cuttack. So that I can able to
serve them as well as perform my duty without any mental stress.

Hence in the above circumstances I pray before your good office to help me in
rendering service to my husband and Father-in-law passing their serious health
crisis and ensure my transfer from Panchasakha Govt. High School Dhenkanal
to any of the above four high schools nearer to my home place i.e. Kandarpur
Cuttack so that I shall remain ever grateful to you.
