AFFIDAVIT REGARDING MARRIAGE STATUS

I, KELVIN SEBASTIAN MSOLA, an adult Christian and Resident of Kibaigwa


Municipality with P.O. Box 80, Dodoma, do SOLEMNLY and SINCERELY declare as
follows:

1. That, I am the above named person hence conversant with the facts I am about to declare
hereunder.

2. That, I am cohabiting with one NJILE CHARLES SHILELE a woman and that we
have one child till now when making this declaration.

3. That, I further declare that wherever and whenever the issue of my Marital Status shall
arise let it be known that my present Marital Status is not married.
